What Does My Lumbar Scan Report Indicate?

Question: HI

I would appreciate if you could take a look at my scan and advise me surgery is advisable ..on neck and back both of which are giving me trouble.

unfortunately the sytem is not allowing me to update this scan so I would appreciate if you could review the scans as I have already sent the scans to some other consultants.

I have gone through your images .
The cervical images show disc prolapse at c5-6 levels with reduced intervertebral space and some pre-vertebral collection.
Is there any joint disease like rheumatoid arthritis?
At present if pain is controlled then you can continue with conservative treatment, physiotherapy, neck collar and calcium supplementations with vitamin D3.
Surgery is indicated in case of severe unresponsive pain and neurodeficit as weakness, urinary complains.
In lumbar region also there is multiple disc prolapse with foramina stenosis particularly on left site.
The same treatment strategy is for low back also as discussed above.
Avoid lifting heavy objects, squatting, stooping forward.
Pain modulators like pregabalin, gabapentin or amitriptyline may be taken.

How would I find out if I suffered from Rheumatoid arthritis? Can this be controlled with medication?

The pain is in the upper arms and in particular if I overuse the arm or lift or put on my cloathing..is is..i am attaching a report from my Neurosurgeon..the neck continues to be very painful.reduced reflexes are globablly.. do you recommend injections for this?
Most of all I would very grateful if you could let me know if I have rheumatoid arthritis..

Hello,
Rheumatoid arthritis is associated with pain, swelling and morning stiffness of hand joint and some time associated with cervical spine involvement also.
In case no such complains present then the Mri is of degenerative disc and bony changes only.
The operative advice may be considered in view of decreased reflex and unresponsive pain.
Injection are short term therapy and not a permanent solution.

I don t have any hand joint issues.. mainly the neck..i would be most grateful if you let me know what is the best injection to get for the neck..i have never had one and am in quite a bit of pain at the moment.also I would appreciate if you let me know do you think it is adisable to have back surgery.. pain in the leg comes and goes

do you think that it is normal for my neck to be causing as muchupper arm pain as it is..it is fairly severe..could this be due to arthritis..mainly when I use arms.
woud it be worth considering opiods for pain relief..do you think they could do a surgery for the neck.. also there are a few compressions in the lumbar spine.. do you think there a too many to operate..multiple disc herinations? is there a solution?.what precisely would be the symtoms of rhuemistism arthritis. thanks

The neck related pain may involve any part of upper limb from shoulder to hand depending upon level of involvement in cervical spine.
Opioids can be used . Tramadol is best to begin with.
Surgery for neck may be delayed as the only deficit is decreased reflex with no motor or sensory loss.
The low back region requires surgery as there is decreased sensation and reflex loss.
The surgery extent is best decided by surgeon depending upon the contribution of different pathology of disc involvement. Sometimes few modifications may be done during operation too.
Rheumatoid arthritis is joint disease involving small joints of hand with swelling, pain and morning stiffness in hand. Proximal involvement in shoulder is not a feature. In some patients cervical spine is also involved. For testing you can get blood investigation like RA factor and Anti-CCP testing.

thanks..when you say proximal involvement in shoulder is not a feature..what exactly do you mean..i get a lot of pain around the front of the neck .. as explained the needly feeling can be dreadful to deal with.. do you think that early intervention surgery would be risky..my surgeon thinks it is coming from the C5C6 disc been severely degenerated. Find attached scan of this disc only

Rheumatoid arthritis do not involve any proximal joint like shoulder; but radiculopathy affecting shoulder region may be secondary to cervical disc involvement in rheumatoid arthritis.
In your case as there is no symptoms related to joint involvement in hand or wrist, diagnosis of rheumatoid arthritis is doubtful. Blood investigations for rheumatoid is R.A factor and anti CCP.

Your C5-6 disc is definitely involved and pain is radicular in nature involving shoulder and scapular region. Surgical approach may give you good benefits.

Discuss my views with your treating doctor.

Many thanks for this. My surgeon has just recommended an injection C5C6 nerve block.. do you think this could help or would a facet joint injection be better. I value his opinion
