Question: My wife had a pulmonary function test early this month. The results: this patient underwent pulmonary function testing for diagnosis of lymphoid interstitial pneumonia with Sjögren's syndrome and reflux. Spirometry demonstates normal FVC, FEV1 and FEV1/FVC ratio. The Post bronchodilator or FVC is 95% and FEV 1 is 86%. There is a marked reduction in the FEF 25-75% mid flow at just 57% post bronchodilator. The DLCO is normal at 97% of predicted. IMPRESSION: Minimal obstructive airways disease with intact gas transfer capability. No bronchodilator reversibility was demonstrated.

From your description, it is apparent that the said patient has Sjogren`s disease. Has LIP been confirmed. LIP is a type of interstitial lung disease which may be a manifestation of Sjogren`s. In this condition patient has involvement of airways and lung parenchyma resulting in breathlessness. However the pulmonary function tests including DLCO are normal except there is reduction at FEF 25-75% revealing small airway involvement. This is a good sign indicating that lung function is not greatly affected at present. Does she also have any allergy/asthma issues as obstructive pattern is generally seen in asthma/COPD.

Dr XXXXXXX thank you for your review. Yes, she has Sjogren's and she has also had been diagnosed with LIP, because of numerous cysts in her lungs. CT scans that were taken 12 years apart show little change, she has no other LIP indicators. But, I know LIP can be very serious. She does not have asthma or allergies but she does have acid reflux.

No progression of disease on CT chest is a good sign.

It is good that the Ct scans of chest do not show significant progress of the disease. Her PFT is also almost normal. The interstitial lung disease(LIP in her case) in sgojren syndrome is supposed to be mild. So lets hope that the disease remains stable and does not affect her quality of life much.
