Question: 15 mm thick heterogeneous endometrial stripe complex is seen.      
This may be from versus endometrial abnormality. Further      
correlation is recommended.      
     
Heterogeneous myometrium with irregular scattered echoes, diffuse      
fibroid disease/mass versus diffuse adenomyosis. If clinically      
indicated, pelvic MRI can be helpful in this patient.

The scan report says that you have a thickened endometrium of 15mm thickness.. This can be normal when you are about to menstruate. The scan also says the myometrium, that is the wall of the uterus has diffused echoes which can be due to fibroid or adenomyosis.. The fibroid is either small or diffuse mass..

I would like to know the reason why this ultrasound scan was performed. What are your symptoms before the scan? from this report, if you have severe period pain, it can be due to adenomyosis and can be treated with pain killers during periods..

As far as management is concerned, if thickened endometrium is just prior to your periods, you can wait for the mensus to occur and by day 4 get a scan repeated to see for endometrial thickness. You can discuss about this option when you visit the gynecologist.

Here are the findings. I have very heavy, very painful cycles. Pain pills do not even touch my pain. I am severely anemic as well. Hysterectomy was recommended 2 years ago but I wasn't ready.

Here are the findings:

Measures 11.7 x 7.4 5.3 cm. The myometrium is very      
heterogeneous with irregular scattered echoes, diffuse      
fibroid/mass versus diffuse adenomyosis. The endometrial complex      
measures 15 mm in thickness and appears mildly heterogeneous.      

Right Ovary: Measures 3.4 x 3.4 x 2.8 cm. There is a simple      
cyst of 2.6 cm.      

Left Ovary: Measures 2.2 x 1 7 x 1.4 cm. There is a slightly      
irregular shaped cystic structure of 11 mm.

Adenomyosis is the cause of the painful periods.. Thickened endometrium might be the cause of heavy bleeding..

Was the scan done when you were due for periods? If yes, then wait for periods and get a scan repeated after periods.. But when you did this scan you were somewhere in the midcycle, you have to undergo a D and C and sent the endometrial sample for histopathological examination..

Since you have a heavy bleeding and painful periods, it would be better to undergo hystrectomy. First wait for a D and C..

Both the ovaries are normal.. Left ovary has a small cyst which could be a functional cyst..
