What Does My Semen PCR Test Report Indicate?

Question: Hai Doctor.I am in treatment of male infertility.Recently , it was found out that SEMEN PCR TEST if positive.Doctor advised to go for TB treatment for 06-09 months. Also he informed me that if we are repeating the test( PCR test ) after the treatment also it will show positive but no need to worry.
Please advice me.

I went through your query and understand your concern.

Tuberculosis still remains a major public health hazard. The primary target of the Mycobacterium tuberculosis bacteria is the pulmonary system in the body. Dissemination to the genitourinary system may occur in approximately 20% of the patients. Pelvic tuberculosis is a silent disease, may be present for 10-20 years without any symptoms. 8-10% of infertility patients may have active TB. The primary site of infection of the genital tract is often the epididymis in men being spread by hematogenous spread. Similar to urinary disease, the infection then spreads to adjacent organs by direct extension. The transmission of genital TB from male to female is very rare despite the fact that men with genital TB can have M. tuberculosis in the semen.

You have not mentioned about your sperm count? Yes if PCR is positive your infertility could be due to tuberculosis. Although hematospermia is a rare presenting symptom of genital TB, TB should be considered in patients who are seen with repeated attacks of hematospermia. Tuberculous epididymitis may be the first and only presenting symptom of genitiourinary TB. The usual presentation is a painful, inflamed scrotal swelling that is difficult to differentiate from acute epididymo-orchitis. Tuberculosis of the epididymis usually presents as a painless or only mildly painful swelling. TB of the testis is almost always secondary to infection of the epididymis via direct extension. Tuberculous epididymitis can also present as infertility due to scarring of the epididymis or multiple vassal obstructions. Molecular methods, like PCR of either DNA or rRNA can detect the bacilli. The high sensitivity (95%) and specificity (98%) of PCR. But tissue PCR after biopsy is a sensitive and specific method for obtaining early and timely diagnosis of male genital tuberculosis. The test can pick up even very small quantities of DNA. Unfortunately, this test has proven to be too unreliable. Because it is very expensive, it has not been validated in the fertile population, as a result of which there are too many false positives.
The tissue PCR results have more diagnostic accuracy in histopathologically reported granulomatous inflammations. You have got semen PCR done which adds qualitative benefit for diagnosing such cases whether positive or not. As your semen is positive it is true that your infertility could be due to TB. You will have to take anti-tubercular treatment for at least 9 months or more. But coming to your main query after treatment your PCR test can be positive yes it can still be positive so need not worry. Also get your wife evaluated for infertility. After proper treatment you stand the chance that your wife gets concieved.

I am well convinced with reply.
Regarding my Semen Analysis, it is showing low sperm count. Sometimes, it is showing within the limit.At present i having no symptoms of TB Infection.
From today onwards, i have started AKT4 tablet for 02 months.
Doctor advised to re-do the Liver function test after 45 days.Based upon that, he will advise further treatment plan.

My wife undergo test for TB ( Specimen test from uterus ) and it is negative. No other fertility problem to her.

Ok you have had shown oligospermia levels in past that suggest probably the cause of infertility could be tuberculosis.Your doctor has been treating you correctly just regularly take akt4 in morning without stopping. Take protein and anti oxidant rich diet with multivitamins and multimineral supplements.Don't eat junk food, don't smoke and avois too oily food. Practice stress relief. Be in follow-up. There are various in utero insemination and IVF treatment available which are affordable and successful.
