I can certainly understand your concerns.
I apologize for the delayed response owing to my work schedule.
I have worked through your query in detail.
You seem to have normal seminal parameters with regard to the ability to father a child.
But few significant abnormalities are also evident which are suggestive of infection.
A short course of urinary antibiotics is indicated in your case.
I generally prescribe my patients with oral pills of Doxycycline for 5 days.Repeat tests after one week. Infection should resolve.

Let history of surgeries even though significant will not hamper your ability to father a child.
