I have seen the semen analysis report done one year back.I don't think it was done from a lab who is frequently doing semen analysis. The normal ranges mentioned are not latest.
Any how ,I suggest you to get a fresh semen analysis from a reputed lab. There should be 3 to 4 days abstinence and wide mouth container should be used to collect with out spillage.
You can upload the result so that I can guide you further.
To improve semen parameters you need to abstain from smoking and alcohol. Eat healthy food , avoid junk foods ,carbonated drinks ,saturated fats etc. Start some exercises daily.
Further tips can be shared after seeing the fresh report.

semen concentration has to be above 16 million /ml
Motility (a+b) more than 50 %
morphilogy more than 4 %
