Question: uterus is anteflexed, bulky and measures approx. 97.6mm x 56.9mm x 64.7 mm. the echo pattern is generally heterogeneous with echogenic mass (haematoma) surrounded by a scanty fluid in the endometrial cavity.
cervix is normal.
adnexae: bilaterally normal.
pouch of douglas: fluid filled.
urinary bladder: normal in size, wall thickness and echo pattern.

Thanks for the query to hcm,
The Ultrasound shows an enlarged uterus with blood collection inside uterine cavity which could be just menstrual blood collected and shall pass out through vagina.
However even though the uterus is bulky , there is no mention of any fibroid per se but if your menses have been painful then adenomyosis can be a cause for enlarged uterus in your case.
The rest of the scan for ovaries and adnexa is normal.

actually i missed my period and was expecting a pregnancy only to see blood 3 days after. could this be early miscarriage?

Hello,
Before even contemplating miscarriage pregnancy needs to be confirmed. you should have got a serum beta hcg test done to confirm pregnancy first. If negative then there was actually o need to but cytotec at all. The bleeding could have been your normal menses.

Please don't be worried.
There is no structural abnormality in the uterus or the ovaries which can deter you from getting pregnant was i can see. just follow your fertile period between day 10-day 18 of your 28-30 day cycle and practice intercourse during that period to increase your chances of pregnancy as ovulation occurs then.
Stress can be a deterrent in conception XXXXXXX may cause hormonal imbalance in women and cycle can get deranged. ONE NEEDS TO BE PATIENT with conception and be positive about it.

The size is bulky but not fairly enlarged. If its an adenomyosis, which can be confirmed after seeing the report ( you need to attach that). But adenomyosis should also not be a deterrent in conception if all other parameters are normal in your and your partners case.

Hello,
The scan shows retained products of conception and hematoma secondary to incomplete abortion which is retained inside the uterus as of now. You never mentioned that you had an abortion and this scan was post abortion, nevertheless the products of conception would have to be removed by dilatation and curettage and then the size of the uterus after the endometrium and clot and Retained products of conception are completely removed , shall return back to its normal shape as Adenomyosis which i was suspicious seems unlikely.

i did not have abortion. i think i lost a pregnancy. i missed my period n had a home pregnancy test which showed positive. 3 days after, i started bleeding and that is why i took this scan. i went through d&c last October with this same problem.

Okay,
In that case you can wait for a week for the clots to pass off naturally and if the bleeding continues even after 7 days then a repeat ultrasound would be required and then a d& c . Else if the bleeding completes in a week then you can consider the abortion as complete and you may not need a surgical evacuation then.

hello Doctor,

Hello,
good to know that your bleeding has stopped. The pains could be possibly due to remaining progesterone effect over the already stretched uteroscaral ligaments and they shall take me days to return back to normal. I suggest a repeat ultrasound pelvis after 3 days to check for completion of miscarriage and no retained products of conception.
In the meantime you may use TAB MEFTAL SPAS twice a day for next 2 days for pain relief and apply hot fomentation to the lower abdomen to soothe the pain.
