The 1st attached report is ultrasound at 3 months
The 2nd & 3rd attachments are dual tests at 3 months.
The 4th & 5th attachments are Ultrasound at 20 weeks.

Query- I want to know about the findings of the ultrasound at 20 weeks. It is written that there is fullness of bilateral pelvis- What does that mean. Is there anything to worry.

Hi dear,
I have gone through your question and understand your concerns.
I have reviewed the reports.
Being a fetal medicine specialist, I can give you appropriate advice in this case.
The ultrasound at 20 weeks show the renal pelvis of both the kidneys to be slightly dilated.
This pelvic dilatation can be present due to many causes like chromosomal anomalies like Down syndrome or trisomy 21, congenital anomalies of the kidneys or can be idiopathic (no cause found).
Since the double marker is low risk for trisomy 21 and no other anomalies are found in ultrasound, the risk of Down syndrome is extremely low.
Such situation should be dealt with serial follow up ultrasounds every 4 weekly to assess the size of the renal pelvis.
If it decreases or remains same, then there is nothing to worry.
If it increases with time, then the delivery should be done in a centre with pediatric urology facilities.

I have few follow up questions. It would be great if you could answer below each question separately.
1) What is the normal size of Renal Pelvis of 20 week foetus for XXXXXXX population?
2) Should we go for Triple marker or non invasive prenatal testing or any other test to find the chances of down syndrome.
3) Are there any case in which NT scan is normal and nasal bone is present but still down syndrome is found in baby.

Hi dear,
1. Normal size of renal pelvis in second trimester is 4-7 mm.
2. There is no use of triple marker as double test has been done and is more reliable serum screening than triple test.
You can go for non invasive prenatal testing, as it gives almost confirmatory test for Down syndrome.
However, if it comes positive, then amniocentesis needs to be done.
3. The sensitivity of nasal bone and increased NT for detecting Down syndrome is almost 75%.
So, it is possible that Down syndrome can be there even if these two findings are normal.

If normal size is 4-7. Then why there is fullness in report as the size in report in left 6 and right 7. shouldn't it be normal.
