What Does My Ultrasound Scan Test Report Indicate?

Question: My wife's a USG report shows 34 weeks 2 days on 30/6/2015. During UsG the radiologist indicated that amniotic fluid is reduced for gestational age with AFI -7cm. then our gynic adviced us to hospitalized for increment of fluid with drips of compund sodium lactate injection IP. During hospitalization they are also perfoming NST. Yesterday they told us that NST report is not so good but ok, today morning after breakfast they had performed NSG and told that it is better today and also told that if it was bad as yesterday then they have to operate. As regarding hospital yesterday iur rooms AC was not working and its also too hot in XXXXXXX so can It effect on NSG.also pleaee advice us what are chances of babys good health and pregnancy till 40 weeks.? Is there any other test also which the doctor have to perform or any thing further we can ask? or any report which i can send to you in follow up?

Hi XXXXXXX I have gone through your question and understand your concerns.
AFI of 7 is definitely less for a 34 weeks 2 days pregnancy. NST gets affected by maternal food intake , hydration status and by the fetal sleep pattern. If the mother has not taken timely meal, is not properly hydrated or the fetus is sleeping (usually for 20-25 minutes at a time), then the NST may not show good results. However, the next step after such a NST is to repeat it after 30minutes-1 hour after a proper meal. If the repeat NST is normal, there is no need to worry, rather the NST should be repeated daily.Moreover, a doppler ultrasound of the fetus should be done to assess the blood flow to the fetus.
If the fetus has not developed any growth restriction and has a good biophysical profile and NST, then there aregood chances of having a healthy term baby. If the AFI increases to 8-10, then you can wait till 40 weeks of pregnancy, but have to get the labour induced at 37-39 weeks if the AFI remains low, NST is deranged or the fetal doppler studies are abnormal or the fetal movements are decreased.
Moreover, to increase the fluid around the fetus, the best method is to drink 3-4 litres of fluid daily, take adequate rest, have protein rich diet, and restrict physical activity.

Hi XXXXXXX thanks for responding. This cardiotocograph strip shows a baseline heart rate of nearly 120 beats per minute, with good variability, and one small deceleration and one good acceleration. In simple terms, it is equivocal CTG and hence a repeat CTG/ NST should be done after30 min to 1 hour for full 20 minutes. Further decision has to be taken accordingly.

yeaterday the USG shows an improvemnt to 7.5 AFI and also good results for NST. Today we got discharged from hospital after consuming two dextrose bottle and a botlle of hermin throughh IPV.
the Dr again called us on 13th july for USG.

We are not in a hurry and want to prolong the delivery date till expected date.
What are our chances for that?
what care and precautions we have to take to reach till expected date?
can we have a normal delivery before expected date in such conditions?

one more thing, we had heard that the hospital (Shanti Mukund ,new delhi) keeps newly born infants in nursery for a time of 24 hours, when i had asked them why they keep there, no one was able to answer that, what is this nursery? why they keep babies their for ? do you have any idea

Hi XXXXXXX it's good to hear that the AFI has improved.
Your wife should take rest, have plenty of fluids and protein rich diet to improve the fluid around the fetus and promote the fetal growth. This helps in prolonging the pregnancy.
Normal delivery can be very well done in such situation, provided the pelvis of your wife is adequate for normal delivery. The pelvic assessment is done at 37 weeks of pregnancy.
Nursery is generally referred to a new born ward where there is facility of oxygen, ventilator and trained staff and neonatologists. Babies who have any complications are kept and managed in nursery, however routine admissions and stay of all babies in nursery is not explainable.
