What Does "peripherally Follicles Located Bilaterally" Mean?
Question: Hello. My ultrasound showed Peripherally Follicles located bilaterally. No free fluid in cul de sac. Uterus Demonstrates 4 mm Endometirum.
I am not sure what this means. I was surprised as I get monthly periods that last 3-4 days and my cycle is between 28 and 35 days.

Thank you for your query. I can understand your concerns.
It is mostly normal as no free fluid and ET = 4 mm are normal.
The peripheral bilateral follicles can be a sign of early mild PCOD but not necessarily so.
Normally PCOD should have more than 12 follicles in each ovary of at least 2-9 mm each.
What do you mean by estrogen dominance ? Do you have reports of your estrogen levels.
If you are asymptomatic with regular periods there is nothing to worry about as of now.

Hello
In regards to estrogen dominance I have had test done to show I have very high estrogen and extremely low progesterone. I also have had elevated prolactin levels ranging from 29 to 46. I have had a 60lb weight gain that dieting and exercise have not fixed. My fasting glucose and insulin are normal. I do have acne but it is mostly on my back. My thyroid is normal. I show a slight iodine deficiency as well. My coristol was high at 729 nmol/l for am and 327 nmol/l pm. I am also severly vitamin d deficient of 16 nmol/l
all these combined is PCOD a possibility?

Dear XXXXXXX ,
It sounds less like PCOD but more like a hyperprolactinemia ( due to a prolactin tumor).
A prolacitin tumor produces high prolactin, high cortisol, can cause acne due to that, and can cause weight gain. It can also cause estrogen dominance.
I will suggest that you get an MRI of the prolactin gland to see if something is wrong.
Follow up with the reports to me.
