Question: I notice in my underwear some very small spots again very small if that . Was hard to tell but believe it's blood from when after I urine over a long period of time. Is this a STD ? I have no discharge or pain

It is possible that you are passing blood from your urethra. This can be caused by a number of different problems, anywhere along the genitourinary tract. Examples of causes include infection as you mentions (STDs, prostatitis, bladder infection even if there is no pain), problems with the kidneys (stones, inflammation), and although it would be a rarer possibility, tumors in this system are a possibility too.

So the thing to do is to go to your doctor or to an urgent care clinic, and get a complete urinalysis done. They should also do a digital rectal to check your prostate. If there is the possibility of STD based on your history, then they will check for that too.

But the only way to know is to go in to have an exam and these tests done.

And even if the spots are very small, it's important to be checked now, because any amount of blood there is abnormal.
