Question: History: Female, 68yr11mo, RA, high BP, high Cholesterol, Mitral Valve repair 2007, diabetic, smoker 52 yr, hysterectomy 1985, gall bladder removed 2011, foot surgery same on both feet 2001: heel spurs, Morton's neuroma, 3&4 toes straightened. Awaiting result of MIBI Dec 19/20, 2016.

Medications: Methotrexate 1cc self injected/weekly; Plaquenil 200mg/day; Candesartan 16mg/day; Folic acid 5mg/6 days; Rosuvastatin 20mg/day; Wellbutrin XL 300mg/day; Omeprazole 20mg/day; ASA 81mg/day; B12 1000mcg/day; D 2000IU/day.

Problem: Left foot: Have had an issue with what feels like internal scar tissue from Morton's neuroma (between 3&4 toes) for several years (like walking on a pebble), reported several times but Doctor never paid any mind to it. Suspect a break recently (3-4 Months) at base of 2&3 toes (accidentally kicked a kitchen cupboard hard playing ball with our 2 small dogs). Had xray, nothing seen. Have episodes of severe burning top of site at base of 2&3 toes. During past 3 days, the toe half of the foot is somewhat swollen (feels huge), skin is shiny, more spread between 2&3 toes, sole scar area feels quite large so that toes don't touch floor when stepping. In total the affected area is the base and toe of #2,#3,#4 toes. It is very difficult to determine if there is discoloration, top or especially bottom. The swelling is alarming. Can you help me understand what is going on, please?

I have read your history and understood your problem.

There is a possibility that you may be having RSD or Reflex sympathetic dystrophy. This can explain the swelling, shiny skin and pain.

I would advise you to keep the limb elevated. Apply ice. Take Tablet Vitamin C 500mg once a day for a month.

Do get an Xray to look for any break in the bones as you suspected. Avoid weight bearing in the painful region.

I have done some reading on RSD and I really feel that is not likely the problem. Is there any other possibility. The pain I am having is definitely not as severe as in an RSD description. It is intermittent (hours could go by, standing, sitting, laying), burning, pinpointed to the left of the base of the second toe. Also, I feel that scar tissue is somehow involved.

Do get an MRI scan of the affected foot. This shall pinpoint the exact cause of the symptoms and if the scar tissue is causing any sort of compression.
