What Does The PET Scan And CT Scan Report Indicate?

Question: Originally had a 3x5x3 cm tumor Cyberknifed 31/2 years ago. Scans since have been PET scans except one a
couple of years ago was a CT scan. Pet scan 4 months ago was good.
CT scan last week, report states "There is a somewhat ill-marginated mass within the lower left lobe" (2 fiducials still there. It goes on to say "this apparently represents neoplasm." There are what appear to be a few brachytherapies seen embedded within the mass". I'm thinking they are the 2 fiducials but this radiologist is saying "a few" (not 2).
Originally after the Cyberknife I was told that the area where the tumor was is now "airspace".

Lung mass with ill defined margins indicate malignancy.It need further evaluation.There may be recurrence of malignancy.
CT guided biopsy can be done for confirmation.Repeat PET scan can be done if needed.

Number of fiducials can be better assessed by CT scan.
It is normal that space created after surgery is filled by air.

It was originally a Adenocarcenoma. If it recurred could it get to be 3.5cm in 4 months?

Would this "mass" be able to be seen on a regular x-ray?

it possible it is something other than a "mass"? Airspace that consolidated?

I understand that the Cyberknife can cause cancer in 2% but normally takes 5-10 years to return.

Two weeks ago I had a metabolic blood test from a Naturpath Physician and was told that whatever was tested hormone wise showed that there was no indication of my body having cancer.

What would you recommend I do next?

Next investigation should be biopsy.Biopsy can only confirm nature of mass.PET scan can also be done.

Aggressiveness of tumour varies.Most likely it is not related to cyberknife.

Consolidation doesn't have irregular margins.There is typical air bronchogram present in consolidation.

Hormone assay for lung mass is not very sensitive.It is only supportive in diagnosis.

Chest X-ray can miss small mass.It is not very sensitive for small lung mass.

You should consult your oncologist.

Could this grow from Nothing to 3.5cm in 4 months?
Could it be airspace that they are diagnosing as ill-margined mass?
Would recurrence show in the same place after Cyberknife as described?
Could it be re-Cyberknifed?
If it were you would you have the lobe removed or Cyberknifed again?
Everything else is stable, no changes.
What could "mass" be other than cancer?

Sorry for replying late.
Aggressive/virulent tumour may grow this much size.
Air space can't be confused with mass.Both have entirely different imaging findings.
Yes,there may be recurrence at the same site.Many times entire removal of tumour is not possible.
Lobectomy or requirement of re-cyberknife depend upon further evaluation.
Solid mass may also be benign in nature.CT scan is not confirmatory.

I think I found the answer.

The scar tissue from the original Cyberknife shows up as a mass. CT scan can not determine the difference tumor or scar tissue.
When I had PET scans they came back negative because PET can tell the difference.
The radiologist that read this last scan did not realize that there were fiducials where the tumor was and thought they were radiation seeds placed in a tumor calling it a neoplasm.
I guess they should take more of a history before reading although I did give them a copy of my last PET scan thinking they could compare the PET from 4 months ago to the scan last week.

Yes,you are right.Quality of radiologist's report is very much dependent upon history and proper sequence of events.
You should try to carry all the relevant reports along with you so that doctors can correlate the findings.
Imaging characteristics of many findings are very much similar.They can be diagnosed better by history.
