I have gone through your query and understand your concern. You are likely to be having osteoarthritis of both knees. Your x ray report is also showing of osteoarthritis ( degenerative arthritis). X ray report is showing calcification ( calcium deposition) consistent with pelligrini Steida ( calcification of medial collateral ligament).Bilateral varus deformities means there is knee is angulated inside in both knee . Large knee joint effusion means much fluid inside the joint . Joint effusion commonly is seen in osteoarthritis. spurring of patella means osteoarthritis of patella.Most of these changes are consistent with degenerative ( osteoarthritis) arthritis. Treatment of it is glucosamine and analgesic such as ibuprofen for pain relief. Quadriceps excercises are helpful. If needed ( not relieved by these measures) you may need aspirations of effusion under aseptic conditions. Fluid can be send for cultural and sensitivity to rule out septic arthritis and other arthritis. You may contact local orthopaedician for it.

Bilateral varus deformity is generally caused by osteoarthritis as it involves first medial( inner) compartment of knee leading to bending of knee in varus. Cortisone causes significant reduction of pain in osteoarthritis. You can return back to hiking, it depends upon pain. There are off and on periods of pain. Glucosamine can slows progression of degeneration and to some extent repair these changes. As it is degenerative arthritis so it is likely to slowly deteriorate with age but it is generally very slow .Surgical options can be discussed after seeing x ray films and if available MRI. One surgical option is Total knee replacement but it is only required if you are having moderate to severe pain and pain is not controlled by analgesics. Benefits of operation is painfree knee. Risks involved are of mainly infection and operation risks.
