Hello!
Thank you for asking on HCM!
I understand your concern, and would like to explain that, facing your displayed blood pressure readings, you need to adjust immediately your high BP therapy, as well as your overall treatment.
Your blood pressure values are abnormally high, and 10 mg per day dosing of Propranolol seems insufficient to control the situation.
So, coming to this point, I would recommend as follows:
First to check a new blood thyroid hormone level, to rule out any possible thyroid dysfunctions (probably a relative overdosing of Thyroxine medication intake, or a potential need to down-titrate its daily dose), as your associated symptoms, like heart pounding, the feeling of being on fire, alternated with icy cold, even your anxiety and excessive stress, raise strong suspicions of a resultant thyroid dysfunction.
Concomitantly, to check some other blood tests like complete blood count (to rule out anemia), liver and renal function tests, as well as adrenal implications on blood pressure (just to check your serum electrolytes, especially potassium).
Don't forget to check your fasting glucose and lipid profile.
After a complete review of your clinical status and the above mentioned lab tests results, It would be reasonable to adjust the antihypertensive therapy (probably to add a new drug from a different class, like an ACEI or ARB, or a calcium channel blocker (if no contraindications exist).
My advice would be to consult an endocrinologist, to rule out any actual thyroid dysfunction and to possibly adjust the underlying therapy, and to consult your internist or cardiologist for a better evaluation of your antihypertensive therapy (increase beta-blocker dosing, add new alternative class drugs, etc).
I think that a careful review of the above mentioned lab tests, as well as a comprehensive adjustment of the overall therapy will resolve these health issues.
Hope to have been helpful to you.
Feel free to ask me whenever you need. Greetings! Dr. Iliri
Which brings me back to my question ! Is it safe for me to take this long haul flight to Palm XXXXXXX California ...on XXXXXXX 7 2015 ??
First, you need to control your BP values.

Hello!
I would strongly recommend you to perform those tests before your planned flight, as your BP values are totally uncontrolled, and the stressful travelling may exacerbate even more.
At least, to perform a blood thyroid hormone level. If abnormal deviation is confirmed, to ask again the medical advice.
Greetings! Dr. Iliri
You are welcome!
