The upper lobe nodule from lung was not described in the previous report. The report suggests, there were no lung lesions.
Previous report has no mention of lung lesions. Not sure, why it is mentioned as unchanged. If the second doctor has looked at the Scans, rather than reports, he might have identified a previously missed lesion (that's only possibility). Earlier doctor might have missed identifying lesions.
The disease progress is present and its changed.

What is prognosis like ?
So the lung lobe upper could be a benign lesion as unchanged plus it did not light up on a pet scan too

Yes, the upper lobe lesion could be benign as it was not evident on PET scan.
Prognosis remains unchanged/ poor as new sub-pleural nodule presence indicates disease progression.
Chemotherapy with additional 5-fluorouracil, leucoverine and Bevacizumab maybe helpful. Your treating oncologist, would be more experienced to manage it better.

Ablation will be helpful, if the lesion is causing compression symptoms. Otherwise, it is limited to removal of lesion. Another lesion, will grow somewhere else.
Chemotherapy is more beneficial to reduce the tumor. It can kill tumor cells, which are growing at other locations too.
