What Does The Following EKG Report Suggest?

Question: On a PST my EKG states normal sinus rhythm T wave abnormality, consider anterior ischmia. Abnormal ECG.
vent rate 70 bpm PR interval 120 ms, QRS duration 86 ms, QT/QTc 390/421 ms, P-R-T axes 33 1 67.
I have been told I need a sign off from a cardiologist before my MD will clear me for bladder surgery. What tests can I expect from the cardiologist ? I hope not a test involving Barium or injecting radioactive solutions. My surgery date is in 12 days.

Hi and thank you for asking HCM!

The first thing that your doctor should do is a complete cardiac examinations and to check by himself your ECG. Keep in mind that an abnormal ECG isn't necessary a sign of disease.

Other examinations are needed to check your heart conditions. If you were my patients i would like to run the following examinations:
- Heart ultrasound
- Stress test
- ECG holter

Only if the mentioned examinations will result positive for ischemic heart disease than i will recommend further examinations which require contrast injection (cardiac CT angiography or coronary angiography ).

what determines a positive for Ischemic heart disease from the above tests ? and how does this effect my going forward for surgery ? In other words, do i have to get a low score on stress test and what presents as evidence of Ischemic heart disease on heart ultrasound and ECG Holter ? thank you

Hi back,

The diagnostic criteria for a positive ischemic heart disease (IHD)include:
- ST segment abnormalities in the ECG
- Miocardial segment motion anomalies (hypokinezia=moving slower than other segments of myocardium or akinezia= no movement at all) in the Cardiac ECHO
- positive stress test for induced myocardial ischemia

Angiography is advised if the test result positive for IHD, otherwise you can run safety bladder surgery.
