I have gone through your father's MRI reports uploaded by you.
Periventricular and subcortical white matter hyperintensities along with lacunar infarcts is suggestive of vascular or multi-infarct dementia.
He needs to be on ecosprin and statins regular dose.Simultaneously,medicines for dementia like donepezil or memantine should be started.

In simple words,MRI says about the brain damage due to poor blood perfusion which may result from the narrowed blood vessels due to chronic effects of some factors like age,high blood pressure or smoking.
He needs to be put on the medicines as I mentioned above to prevent further brain injury and to help improve the memory.
He also needs to be screened for high blood pressure.
