Question: How can I get relief from Sciatica pain (right side) and how physiotherapy help me? there was some discomfort and small amount of pain in my lower back for last 25 years. In August, 2004 it was un bearable. MRI report and Doctors’ prescription of that time attached. From July, 2016 I am facing same problem. Latest MRI report and Doctors’ prescription of that time attached.

Hello,
I have gone through your question,reports and understand your concerns.
You are having multiple disc prolapse with significant compression at L5-S1 disc causing symptoms towards right side.
The management of disc related radiculopathy is medical and then surgical.
Preventive and precautionary steps should be taken.
Avoid stooping forward, lifting heavy objects, squatting, sitting crossed leg.
Lumbar belt can be used if pain severe.
Physiotherapy under physiotherapist guidance should be done regularly to strengthen the back muscles.
Medications are pain killers with muscle relaxant like etoricoxib or Aceclofenac with thiocholchicoside or tizanidine.
Pain modulator drugs like pregabalin or amitryptiline or gabapentin can be used.
Dosage of medications are titrated gradually.
Surgical options are available if required.
endoscopic and open surgery both are available.
Trial of methylprednisolone or Epidural analgesics are available too.
In case of unresponsive pain, deterioration in power, reflex loss or urinary incontinence, surgery is planned.
