What Does The Following Abdominal Ultrasound Report Indicate?

Question: I just had an ultrasound of my abdomen. I had surgery in August 2016 due to bladder cancer. I now have an Indiana pouch. My ultrasound reads that there is fluid collection in the right lower quadrant would correlate with the known pouch. There is a trace free fluid in this area and mild left pelvicaliectasis. Findings consistent with fatty infiltration the liver. I have been experiencing moderate to severe abdominal pains and my abdomen is distended and tender to touch. It is very warm to touch on the lower area of my pelvic region. When I catheterize it is somewhat painful with blood in my tubing. I am getting approximately 200-300 cc of urine at a time and have bee doing this every 2-3 hours because of the fullness I feel. I haven't heard from my surgeon on what is going to happen and I saw my doctor yesterday and I don't think he knows what to do with me. Please can I have your opinion on what is going to happen. Thank you.

Patients who have an Indiana pouch run the risk of infections from the stoma, difficult catherization, pain around the stoma, stones, and leakage. Also this type of urinary diversion causes immediate metabolic changes that can give a wide range of symptoms from diarrhea, , electrolyte abnormalities, hepatic metabolism, and possible bone health deterioration. Over the lifetime of the Indiana pouch, the recipient should have their kidney function closely monitored

In this case i will suggest you

1-Urine analysis
2-Uroculture
3-Meeting with the surgeon to control the device

In my opinion we are dealing with an infection that requires treatment
