Question: I need to know what my brain mri means:" There are several tiny signal hyperintensities consistent with tiny ischemic foci of microvascular occlusive disease involving predominantly the subcortical white matter of both cerebral hemispheres. There are mild atrophic changes involving both upper cerebral convexities, but no other abnormal signal, mass effects or ventricular or cisternal abnormalities are noted involving the ecalvarial contents. There is no siscernible acute infarct, intracranial hemorrhage or space- occupying mass lesion. There are grossly normal arterial signal voids at the base of the brain. Asso the left side of my face and head get numb when I take meds that my nuerologist has prescribed? Any ideas about that? And is it safe for me to have surgery to free up pinched nerve in shoulder, i.e. is it ok to be anesthesized for my brain during operation? Thanking you for your careful consideration of this question. I am a 67 year old psychotherapist who has been hit in the head as an adult accidentally by objects falling on me. As a child I was beaten about the head. I have much memory loss. Does this mean that I will have alzheimers?

Brief Answer:
Your MRI shows mild aging changes in brain and is not a concern

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find the interpretation of your MRI scan in simple language below.

1. There are several tiny signal hyperintensities consistent with tiny ischemic foci of microvascular occlusive disease involving predominantly the subcortical white matter of both cerebral hemispheres.

This is an age related finding and can be expected at your age. This says that the blood flow in brain through small blood vessels shows reduced flow. As a result small areas have evolved where there has been slight irreversible brain changes due to lack of blood flow. This is showing that the blood flow is reduced slightly and is not a serious concern.

2. There are mild atrophic changes involving both upper cerebral convexities, but no other abnormal signal, mass effects or ventricular or cisternal abnormalities are noted involving the calvarial contents.

This says that the brain markings on the surface have become more prominent and are as it should be at your age. The brain fluid spaces are normal and the pressure inside the brain is also normal.

3. There is no siscernible acute infarct, intracranial hemorrhage or space- occupying mass lesion. There are grossly normal arterial signal voids at the base of the brain.

This means that there is no tumor or hemorrhage in the brain. Blood supply in the brain is also otherwise normal.

In conclusion your brain is showing mild aging effects and is not a serious concern. Please do not worry.

The numbness on the left side of face might be due to a pinched nerve in the neck region and that has not been assessed on the is MRI scan.

It is safe to get anaesthetized for any surgery including that of a pinched nerve in the shoulder regions as long your heart and lung functions are alright.

You will not have Alzheimers. The incidents in the past including childhood and falling objects on the head has not caused any concerns. Your MRI can be considered as expected in any person at your age with signs of mild aging. Please do not worry.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek
