What Does The Following Echocardiography Report Suggest?

Question: Hello Doctor
My brother aged about 56 years is suffering from holes in heart with the final impression of Echocardiography report as CHD-PM VSD (10 mm), Bidirectional shunt, Moderate MR, PR, Trivial AR, Severe TR, Moderate PAH. Kindly advice on this and any Ayuvedic treatment/ medicine for its recovery.

Thanks for contacting Ayurveda with your health concern and following is/are recommended guidelines:

1. Since there is presence of congestive heart failure and Pulmonary Hypertension (CHD with PAH) alongwith valvular pathology thus prior to management aspect do let me know about following symptoms:

FEATURES OF MITRAL REGURGITAION
a. exertional dyspnea, orthopnea, fatigue, palpitations?

FEATURES OF RIGHT/LEFT SIDED HEART FAILURE
b. swelling in feet,mildly enlarged liver, engorged neck veins?

RHYTHM
c. Pulse Rate? (whether regular or irregular)?

FEATURES OF PULMONARY HYPERTENSION
d. Chest pain,cough,blackouts, blood in sputum, breathlessness,dizziness?

2. Do let me know whether apart from ECHOCARDIOGARPHY report any other investigation (particularly ASO Titre) has been carried out to exclude any rheumatic origin.?

3. Do let me know about the treatment which he is already undergoing for better drug-drug interaction (if willing to take Ayurveda herbs with Modern drugs)

4. Treatment (at this stage) is usually of complications, mainly (heart failure, atrial fibrillation and SBE [subacute bacterial endocarditis]

Sir,
Thank you for your advice. I am sending herewith the additional medical test reports as desired for further conclusion and suggestion.

Doctor! The following symptoms surface sometimes-
-Fatique after prolong work/ walking
-Sometimes swelling in leg and face, again it disappears after sometimes
-Mild chest pain
-Sometimes disturb free passing of urine
-Pulse rate normal
-No problem in taking food

Dear
Thanks for reverting back and going by your case history and investigations following can be inferred [not concluded]:

1. Since there is absence of any vegetation thus rheumatic cause is excluded
2. since there is no evidence of clot/embolism thus any medical emergency is excluded [till date]
3. Since in other Investigations there is prescription of anti hypertensive medicines thus probable cause of the pathology could be " hypertensive heart disease" causing 'Hrid Roga' [heart disease] i.e. valvular incompetence [ kapata daurbalya]
PS. NYHA II in Prescription.

4. Since there is on & off presence of symptoms [fatigue/swelling/chest pain] thus related to disturbance of normal structure of heart [mild/moderate/severe valve incompetence]
PS. since valvular problem is MECHANICAL thus best treatment is SURGICAL, Ayurveda herbs are best given as PROPHYLAXIS for
. improvement of valve function
. delay heart failure
. prevent chronic/long term complications

5. DLC Report is abnormal along with low S. Calcium [what was the opinion of Cardiologist]

6. Make sure to remain under constant observation and supervision of your attending physician/cardiologist and if interested in taking alternate system of Medicine you should inform your doctor, and let me know the same so that the Dietary Regime- Lifestyle Management- Medicine can be initiated from my side.
