What Does The Following Kidney Ultrasound Report Indicate?

Question: my son has a uvj obstruction on his right side. he is currently 18 months old. 6 months ago a renal scan showed 50-50 function. but yesterday the same scan showed his right kidney functioning at 30%. he has surgery on auhust 29 to remove obstruction. my question is once surgery is done will the function improve back to normal?

Hi,

The case is of Uretero vesical junction obstruction. It can lead to back pressure changes because of obstructive flow and lead to kidney damage.

So first thing is to prove if there is obstruction. this can be seen in ultrasound as hydroureteronephrosis and as obstructive flow on DTPA scan. if both test confirm obstruction then operation to releave obstruction should be done.

now if the size of this kidney has become smaller or if the patient had urine infections then the recovery of kidney funtion may be less. but if kidney sizes are good then it should show some recovery. how much recovery will occur can not be predicted and it may need a reassessment after 1 month.

The recovery should be judged by improvement in absolute kidey function on DTPA and not by percentage function.

does hydronophrois increase the likly hood of have problems when he gets older. is is only 18 months now. thanks

Hydronephrosis causes kidney failure in the long run

Detailed Answer:
Hydronephrosis means dilatation of the urine collecting system. It may be from reflux nephropathy or obstruction. If it is by obstruction and we do not do any operation treatment, then the kidney may fail slowly over years. Also, it increases risk of urine infections.

If the obstruction is removed now by operation then the kidney damage may stop further or may even reverse towards normal.

he had 50-50 function for his first year of life. then at his 18month appointment is right kidney was only functioning 30 percent. once surgery is done what would you expect funtion to return to based on your experience? thanks

if the kidney is obstructed and not treated. slowly the function will become almost zero in that kidney over years. also there will be risk of urine infections.
