What Does The Following Liver Function Test Report Indicate?

default
Posted on Thu, 13 Nov 2014
Question: Could I please have an interpretation of the rate of hepatic decline for the following 3 days (unfortunately, the pathology results for Day 3 are not as complete)?
DAy 3: ALT 21 U/L; AST 29 U/L; ALP 133 U/L; bilirubin total 77 umol/L; albumin 35 g/L; protein 69 g/L; Gamma GT 76 U/L; PT Test 21.7 sec and INR 2.0
DAY 2: ALT 16 U/L; AST 25 U/L; ALP 101 U/L; bilirubin total 48 umol/L; albumin 28 g/L; protein 59 g/L; Gamma GT 55 U/L; PT Test 22.4 sec and INR 2.1
DAY 3: PT test 23.2 sec and INR 2.2.
How serious does the hepatic dysfunction (failure?) appear to be overall? Also, despite the incomplete results for DAY 3, is it possible to say that the hepatic dysfunction (failure?) was any worse on DAY 3?

Brief Answer:
significant liver dysfunction

Detailed Answer:
Hello
Thank you for the query.
Well, I understand your concern.
Looks like you have significant liver dysfunction as the INR is deranged. However the overall trend has been static as INR is almost static.

You have mentioned two reports as day 3, mostly you must have mentioned 1st as day one I guess. In that case there is some decline in bilirubin which might suggest some degree of improvement in liver function.

So need to consult your gastroenterogist for same for optimisation of your heart Medications, possibility of giving three doses of vitamin k injections to improve INR. Also need to avoid medications like painkiller and other over toxic drugs and alcohol of any.

I would suggest monitoring liver function tests once 3 or 5 days as daily monitoring is unlikely to benefit.

Hope this answers your query.
Wish you a speedy recovery.
Happy to help.

Thank you Dr Deshmukh for your advice! Regrettably, I mistakenly referred to DAY 1 as DAY 3. It is supposed to go from DAY 1 to DAY 2 Day 3, as you suspected. You mentioned the possibility of administering Vitamin K. Would it be the normal practice to administer Vitamin K by IV? Is this a safe route? Also, in your opinion, is it more accurate to say that there is a hepatic impairment or a renal failure?

Vita min k can be administered iv special preparation is available.
