Question: Semen analysis in the Month of October 2016

Time of collection. 9:30 am ( delayed liquifaction
Period of abstinence : 5 days
Quantity : 2 ml
Appearance : opaque
Odour: XXXXXXX
Reaction: alkaline
Fructose test: positive

Total sperm count: 25 million per ml
Motil
On collection : 30 percent
One hour after collection : 25 percent.
Total abnormal sperm: 30 percent
Pus cell: nil
Red blood cells : nil.

Thereafter I have done a Semen Analysis in February 2017

Time of collection. 8:40 am
Period of abstinence : 3 days
Quantity : 2 ml
Appearance : opaque
Odour: XXXXXXX
Reaction: alkaline
Fructose test: positive

Total sperm count: 25 million per ml
Motil
On collection : 45 percent
One hour after collection : 30 percent.
Total abnormal sperm: 30 percent
Pus cell: nil
Red blood cells : nil.

We have been trying to have a child since last one year. I have also done Semen Culture and Sensitivity on 14/Feb/ 2017. The said test showed staphylococcus. I have visited the doctor who has given antibiotic ofloxacin for 5 days from 15tth Feb night.

Kindly advise me on the said reports. There has been an improvement in the motility from 30% to 45% prior to commencing the antibiotics.

I have gone through your query. The count is ok although on the lower side. The motility too is not very good. Not bothered about the percentage of abnormal sperms as this percentage will actually vary with the threshold they define (using typical criteria, anything more than 4% would have been considered ok, but in such a case the normal percentage would not have been expected to have gone above 10%).

I would insist you to take Folvite tablet 5 mg once daily (folic acid) and Cobadex CZS tablet (contains zinc) once daily for the next 3 to 6 months. Folic acid and zinc supplements have been proven to improve sperm count and motility.

Other than that, have unprotected sex on every alternate day with your partner between the 10th and 20th day of her cycle. This will ensure an optimal number of sperms.
