What Does The Following Semen Analysis Report Indicate?
My wife did HSG test on february 2016. HSG test was normal and there was no block in the tubes at that time. Then in XXXXXXX 2016 she got conceived naturally. But she had a miscarriage in 2.5 months and there was no heartbeat so we did a D&C. After 3 months of D&C my wife developed mild diabetes and she is now taking metformin 500 mg twice daily. She also got mild thyroid at the time of pregnancy but it is under control now. Now we are planning for IVF beacause of age. My wife is 31 yrs old and my age is 37. We are married two years before. Is it advisable to go for IVF or should we wait to conceive naturally. My semen analysis is 18 million sperm count and 1% normal forms. My wife's blood test and all other tests are normal. Also the doctor said to take HSG test again for IVF treatment. Is it mandatory to take HSG test again? Please advice us.

Hi, I think the main problem which is causing concern to me is your semen analysis. You told you have only 1 % normal forms. That means you have severe teratospermia. In this, the chances of miscarriages are higher. So, it can be one of the reason of the previous miscarriage. So, I think you should get yourself examined by a urologist. It can be due to some varicocele. Also stop smoking if you are a smoker. You can try some medicines like clomiphene to improve the teratospermia. Get a repeat analysis done along with a semen culture and sensitivity test. If there is no improvement, you have to undergo IVF with ICSI. Now regarding the HSG. It is generally done to see the tubal patency. So, in IVF , it is not mandatory to do it as there is no effect of tubal patency on IVF. Discuss with your doctor regarding this.
