What Does The Following Semen Analysis Report Indicate?

Question: my semen analysis is
ph=8
color=opaque white
viscosity=normal
qty=2ml
count=75mn/ml
motile=50%
active motile=10%
sluggish motile=40%
non motile=50%
morphology=normal in 70%
pus cells=1-2 hpf
i am taken paternia plus one daily for 4 months and now i am taking paternia xt one daily for last 1.5 months
pl advise as my wife is not getting pregnant

Detailed Answer:
Hi, I think your semen analysis report is normal except for a lower active motility. If you have taken paternia plus and paternia Xt, you can get a repeat analysis done. Also do a semen culture and sensitivity test, because sometimes infections can lead to decreased motility. You can take some antibiotics like doxycycline 100mg twice a day for 10 days empirically. Discuss with your doctor regarding this. Now, regarding your wife, I think if you have tried for more than a year, she needs evaluation. Do a thyroid profile and prolactin levels and a ultrasound for her uterus and ovaries. Track her follicles growth by repeated ultrasound to find if ovulation is happening or not. if there is any problem, it can be treated. Also do a HSG to see the tubal patency. If everything is fine, you can go for ovulation induction with timed intercourse for 3 months. If it doesn't work, then you can go for IUI. Discuss with your doctor regarding all these options.

Hi,

I think if your wife periods are regular and all other reports are normal, you can try naturally for 3 cycles. Track the ovulation by repeated ultrasound or ovulation detection kits and be in contact with your wife every 2 to 3 days after her periods stop. It will increase chances of your pregnancy.

If it doesn't work, then you can go for ovulation induction with IUI. In this, your wife will take some medicines like clomiphene for the growth of follicles. Track follicles growth by repeated ultrasound and when follicles is more than 17 to 18 mm, injection will be given to rupture the follicles and IUI will be done the next day. In IUI, your semen will be processed and count and motility will be increased to a great extent. The processed semen will be instilled into your wife's uterus. Progesterone will be given for next 2 weeks. Pregnancy test will be done after that. you can try like that for 3 to 6 cycles. The chance to get pregnant in one cycle of IUI is 10 to 15 percent only.

So, if it doesn't work, then the option left is IVF.
