What Does The Following Thyroid Ultrasound Report Indicate?
I have a thyroid nodule on the right lobe, noted about two months back. Physician had advised ultrasound scanning. I went for ultrasound scanning. The scan report says:
Right lobe thyroid enlarged with a nodule measuring 55x28x36mm with increased vascularity. Tiny calcific nodules. No cystic areas.
Left lobe normal in size and measures 45x10x21mm. Normal echotexture. Normal blood flow. A tiny nodule of 3mm noted towards the lower pole.
Isthmus normal.
Few discrete cervical lymph nodes on both sides.
Impression: large solid nodule on the right with tiny calcifications.
Thereafter I have consulted a surgeon and undergone FNAC. According to FNAC report:
The smears show Moderate to scanty colloid . Benign thyroid follicular cells are cuboidal in shape.Few histiocytes are also seen. No malignant cells seen.
Though the FNAC report says no malignancy; surgeon asked me to undergo surgery .I would like to avoid surgery.
I have been taking medicine for hypertension (losacar 50mg daily) and have severely cut down salt intake for the past 10 years. No other medical problems/treatments .
Is surgery the only option?? Please advise me if it can be cured by medicine. I have no problems with this nodule .I am 59 years old, male, weight 62kg, height 5,3’’. I am in Trichur, XXXXXXX XXXXXXX

Welcome to HCM. I had gone through your question and understand your concerns.
Though the FNAC report says the swelling in not cancer it is better to undergo surgery. This is because the chance of cancer increases when the size of the nodule is more than 4 cm (40 mm). The reliability of FNAC in cancer is only 60 - 70%. Nodule of this size is unlikely to shrink with any medical treatment. So when the nodule is more than 4 cm we prefer surgical treatment. The chance of cancer is also higher in males.
The low salt intake may not have influenced the thyroid nodule if the thyroid function is normal.

I have read through your advice and I am positively considering surgical removal of the thyroid nodule.
I am a teacher by profession and I belong to Alwaye, Ernakulam district , though settled in Trichur.
Are there chances of recurrence,change in voice,calcium loss etc after surgical removal of thyroid??
With these apprehensions, I request you to please suggest the specialists/hospital facilities available for carrying out thyroid surgery in and around Trichur/Ernakulam.

I understand your concerns. The risk of these complications range from 3-13%.
The complications depends on the expertise of the surgeon. You can have it done with the help of an experienced surgeon or a super specialist - Endocrine surgeon.
Near to your place, good centres include XXXXXXX Institute of Medical science where there are general surgeons / head and neck surgeons or the hospital where I am working - Silverline hospital, Kadavanthra where we have an Endocrine surgeon.
