The ultrasound your wife shows multiple small follicles in left ovary and normal right ovary. However, no dominant follicle is documented even on 14 th day of the cycle, which is highly suggestive of anovulation.
Multiple small follicles in the ovary and absence of ovulation makes the diagnosis of polycystic ovarian disease likely. In this condition, conception is a bit difficult.
The treatment for infertility in PCOD is stepwise and includes ovulation induction drugs, intrauterine insemination and in vitro fertilization as a last resort.
Your wife should also take folic acid tablet daily as taking it in the preconceptional and preconceptional period decreases the risk of congenital anomalies in the fetus.

Hi XXXX, thanks for responding. Average duration of treatment of infertility in PCOS is not defined, however generally 6 months of ovulation induction by drugs is tried before going for intrauterine insemination.
