I have gone through your query and understand your concern. The EMG/NCV study is suggestive of moderate nerve (Median) compression at wrist leading to treatable Carpal tunnel syndrome. This leads to numbness and tingling sensation in thumb, index finger, difficulty in doing fine work and muscle atrophy of hand.
The definitive treatment is small surgical procedure (day care) in which the tissue causing nerve compression is released at wrist. Although the nerve study test is suggestive of moderate nerve compression, it needs clinical examination to have any decision regarding role of surgery.
Does he smoke or take alcohol?
My suggestion is to have blood test for blood sugar and vitamin B12.Meanwhile he may have drug like pregablin and vitamin B12 for few days to see any improvement.

I up loaded the nerve test. His tricep and should hurts him really super bad. The injury was a year ago. He did have therapy then with little relief. It weird theres nothing about his tricep or shoulder showing any nerve issues

You are welcome. I have gone through your reply carefully and would like to know about the range of movement at left shoulder as compare to right shoulder?
The radiating pain at shoulder and arm could be because of pinched nerve at cervical spine and this may be a separate issue altogether from carpal tunnel syndrome (median nerve compression at wrist).To confirm pinched nerve MRI cervical spine is the investigation of choice.
To sum up, my suggestion is-
1. MRI cervical spine along with blood test for vitamin B12 and blood sugar level.
2. To reduce his suffering he may have drug like pregablin (prescription only) along with muscle relaxant thiocolchicoside and pain killer Ibuprofen will help. Discuss about all these with your doctor.
3. Wrist brace is recommended to reduce symptoms in hand.
