What Does The Non-contrast Computerized Tomography Report Indicate?
Question: As per NCCT KUB report, my wife is having 6.3 * 4.5 mm calculus in the right ureter at S1 vertebral level. 1.Can this be expelled out by drinking lot of water without medication or operation. My wife has back pain which is not very severe.
2. Also please suggest for stone of this size should we try expelling it out by medicine or go for operation. We want to avoid any kind of operation/surgery as much as we can.
3. In case operation is required I believe, this can be treated without PCNL , I mean by operating through urethra and getting stone out of ureter via urethra .
4. There is a swelling in kidney. I believe this is because of obstruction of urine by stone in urethra. Will there be any risk becuase of this kidney swelling if we try out first by drinking lot of water and medication initially. How much time we have to try this option before considering other options like getting stone out of kidney via operation.

As an Urologist,i can understand your concern.
It's good you've sent the CT scan report,so it's easier to advise.
I'll answer the questions you've asked,in the same order.
1.and 2. As the flank pain or colic,isn't severe,she can very well try medical or
conservative treatment initially. However,just drinking water or fluids,won't
help..
She must take Potassium Magnessium Citrate-B6 tablets or syrup,twice daily,along
with 3 liters or 15 glasses of fluids daily.
The medicatiion is to help dissolutiion of the stone and help in expulsion.
This can be tried for about a month and then repeat the scan.
3. As position of stone is in the ureter,PCNL is out of question.
URS- lithotripsy is standard treatment,if needed. PCNL is for kidney stones.
4. You're right that the swelling is due to the stone,blocking outflow of urine
from kidney to bladder,in the tube called ureter,not urethra.
Repeat the scan after one month and confirm if the stone has flushed out.

One last thing I want to understand,
The position of stone as specified in CT scan is "calculus in the right ureter at S1 vertebral level". Is this just outside the kidney or upper half of the ureter or lower half of the ureter. Just want to understand where we can think the stone is situated in the ureter based on this report.
