What Does The Thyroid Ultrasound Report Indicate?

Question: I had a thyroid ultrasound as a workup for pheochromocytoma. The US showed a 8mm solid indeterminate nodule and a small complex cyst. Could this be associated with the symptoms of a pheochromocytoma? I have elevated urine catecholamine levels (2-3x) and HTN with exertion (220/140) that quickly returns to normal (110/70) and palpitaions. What is the recommended treatment or followup?

Good day,
noted your concern.
Your doctor ordered thyroid ultrasound to look for medullary thyroid cancer and parathyroid nodules . They can be part of MEN syndrome ( multiple endocrine neoplasia). But please remember that, majority of nodules seen during ultrasound in thyroid are NOT cancer.

After the age of 40 years, it is very common to have such nodules and most are harmless. But having seen them, especially in view of suspected pheochromocytoma, the next step in you would be to do a serum calcitonin level and calcium levels. This is to rule out medullary thyroid cancer or parathyroid nodules which can co exist with pheochromocytoma.

We need to work up for pheochromocytoma. You will need a CT scan of Adrenal gland and a nuclean scan ( MIBG scan). Further decision is based on this.
