What Does This CT Scan Of A Cervical Cancer Patient Indicate?
Question: My mother diagnosed with Cervical Cancer stage 3B.
We have taken 2 opinions, they both differ so need your inputs,
Opinion 1 - Oncologist/ Radiologist suggested 5 weeks, Monday to Friday EBRT/IMRT radiation. Chemo therapy (drug name - Abraxin) one a week and 2 brachytherapy after 5 weeks of radiation is complete.
Opinion 2 - Doctor suggested this cannot be controlled because cancer has spread to axilla.
I am not sure how cancer can spread to axilla when it is locally advanced.
Questions:
- Can you please check reports and let me know if this is controllable/ curable?
- If it is controllable then can you suggest detailed treatment plan? next steps?
- How do I help my mother to control symptoms if there is no suggestive treatment plan in this situation?
- Do you suggest any alternative therapy?

Dear XXXXXX,
Sage 3 B carcinoma cervix means locally advanced disease. It is very unlikely that it has spread to axilla, however this can be confirmed by simple FNAC Test.
In stage 3 B the best method of tratment is by giving External beam radiotherapy with single drug cisplatin once a weak to be as radio sensitizer. This treatment is given ideally for 4 weeks which will take care of the local disease as well as the pelvic disease. 4 weeks gap is given after ERBT and pt. is planned for intracavitary brachytherapy. This is given in two sessions.
Latter We need to to do a PET scan of whole body to see if there is any residual disease or there is any distant spread. If PET scan is normal we need to keep her only on follow up. If any focus is found on any part of the body, it can be treated by local radiation or if the disease is multifocal then definitely chemotherapy is indicated.
The protocol will be decided after evaluating the patient and make an opinion which protocol will be useful to her.
