This type of problem including degenerative changes and associated NEURALGIA is believed to be caused by 'Vata' vitiation.

Hydromyelia or syringomyelia is because of a cyst formed within spinal cord that elongates and damages the centre of the cord, thus affecting connected nerves. Modern science is unclear about causes but this is covered under 'apbahuk' like diseases in Ayurveda.

1. There is very clear cut Ayurveda treatment to begin with. Decoction of the leaves of Harsingar plant, the night flowering Jasmine tree, should be started immediately. It's commonly available in XXXXXXX Even extract of leaves may be given. Decoction 1 to 2 glasses may be given twice a day. If not available Panchakol powder decoction may be started as soon as possible.

2. XXXXXXX Asav, Maharasnadi kwath both liquids , 10 ml + warm water separately twice daily. Preferably Asav after meals and Kwath before meals.

3. Tab Rumalya forte, mahayograj guggul each 1 TDS with milk or warm water ( mix turmeric powder in water or milk 1/2 tsp)

4. Vishtinduk vati 1 bid with milk

5. Kanchnar guggul 2 bid ( this is esp for hydromyelia)

6. Mild herbal purgative e.g. Tarunikusumakar chooran 1 tsp at night for three days initially. Then once a week.

7. Mahanarayan oil + mahavishgarbh oil mixed, gentle massage/ application over neck and do hot fomentation over it post massage.( please note : do not press/ compress during massage. It may be harmful. It's to strengthen the musculature and manage pain)

If you do not get Harsingar leaves, or any other, pl go with rest of medicine. These are harmless, safe, no side effects. No interaction with other drugs.

You will find relief from two weeks onwards. The response will determine further assessment of days. Cyst may take longer to cure.

Physiotherapy and Yoga postures (mild) are beneficial. Deep breathing will also benefit. Later, for complete cure Panchakarma may be undergone at some good Ayurveda hospital.

Cervical Region problem, nerves emanating from C5 to C7 area are affecting you. Since spinal cord is also affected due to cyst. Ear pain can also be due to nerve connecting with brain. Spinal Cord is already affected. You have not reported any other ear problem like discharge etc.

Vishtinduk Vati already suggested will take care of ear pain too. At the most, additionally, you may put Ayurveda's Panchaguna oil or Bilvadi oil drops in ear. These are indicated for ear problems too. I recommend either of the oil in order of preference Panchaguna or Bilvadi. You may get medicines in UAE also. Any manufacturer's medicine is acceptable so far as the medicine is same. In case any other medicine of the group is advised there, please ask with mention of ingredients.

Yes you can go for physiotherapy under expert's guidance.

Harsingar decoction is not available readymade. Pl inform someone before coming, to ask some XXXXXXX (gardener) to arrange for you. You may explore net also. Depending on your location, you have to try. These leaves are common in sub Himalayan range/ North XXXXXXX - XXXXXXX has plenty, but may get in other parts too on trying. You will have wonderful improvement after consuming this decoction.

Kanchnar guggul is inevitable. I want you to start it. Till then pl take Rumalya forte capsules of Himalya co. Same qty.

Tarunikusumakar chooran is a mild purgative that cleans gut. However Triphala capsules of Himalya co. may be taken 2 bid initially and then 1 bid after few days. Dose is not fixed. But take it continuously.
