What Does This MRI Report Indicate?
Question: 31 Year old male.
About 2 months ago I developed a vestibular/tingling issue.
What happens is for about 1-3 days I will get bi-lateral facial tingling, mostly on my cheeks. Not severe, but moderate and annoying. This is like a creepy/crawling feeling, tingling, numbness. No loss of facial movement or strength. That will last about 1-3 days 24/7 then slowly subside, at which time dizziness then begins. The dizziness is not a true vertigo, but more of a motion sickness, just got off a boat/roller coaster feelings, with some lightheaded feeling at times, being pushed to the side feeling, etc. I do get minor photophobia with it at times. This will last for daysâ¦. 3-9 days so far. It will then usually end, and I will get a couple close to normal days with near no symptoms, then the cycle starts againâ¦.. Facial tingling for days, then the vestibular symptoms.
It seems that during the vestibular part, when I lay down or rest I feel much better then. I feel great during sleep. Going in a car/ grocery store, movement, makes it worse.
I had a recent MRI, after about 2 weeks after this started and it was clear. (ATTACHED). I have had 2 in office nuero exams and they were 100% normal. I had a test done on my ears at a vestibular lab which was normal for inner ear function. However my rotary chair test was abnormal he said, which points to a brain process in his option.
So far the doctors have told me that because my MRI is clear, ear tests normal, that in their opinion I am having âVestibular Migrainesâ. I will note that during these episodes I have yet to have a ârealâ headache, but maybe a minor one at times.
I do have a past history of undiagnosed all day long headaches that happened from about 2009-2011, then went away on their own, but since 2011, I have been pretty much headache free until this new onset of âVestibular Symptomsâ have hit.
The only other clue I can think of is I always noticed thought-out my life that I could not tolerate spiny amusement rides, and sometimes visual effects, like going into a grocery store would make me feel lightheaded in general.
My question is, what is your option on this âvestibular migraineâ ? Is there something that they could be missing that I should be tested for? I was thinking maybe I have MS because of the past symptoms, a couple years break, and now again? But they said no way with clear imaging and normal MRI. Lyme disease?
Is there anything else I can look into? I donât like how my face tingles for DAYS.. I have read in migraines where someone can get facial numbness for like a hour or two, but have you ever heard of this happening for DAYS? And then vestibular symptoms following?
They want me to try a migraine prevention medication, but I am un-sure on the diagnosis and feel they could have missed something. But maybe not? Need opinion.

According to my opinion, you may probably be suffering from vestibular migraine with intermittent episodic vertigo and sometimes headache and photophobia.
Possibility of multiple sclerosis is unlikely without any positive MRI or objective neurological deficits.
You can very well try migraine medications for few weeks and see the response.

Thanks for response. I am going to try the migraine prevention medication. In the case that I do not respond are there any other differantional diagnoses or tests I can look into for further investigation? thanks
