What Does This MRI Report Indicate?
I started getting back pain 4 months back. It triggered after I traveled to a place in a day and after coming back started feeling pain on lower back. I work in a Software company and work around 8 hours daily.
I got MRI done but Doctor says its minor problem will get resolved. I took Physiotherapy sessions around 30 till now. Doing core back strengthening excersices prescribed by them.
But after 4 months the problem has reduced but still its difficult to sit at work and drive my car.
I have consulted multiple doctors but no one has clarified the exact problem.
I am looking for advise to know my exact problem. Below is the high level MRI report.
Lumbar Curvature is mildly straightened.
I/V disc shows normal signal intensity.
No significant diffuse disc bulge noted.
Sagittal diameters of the thecal sac at disc levels ( in mm) as follows
L1-2 13mm
L2-3 11 mm
L3-4 10 mm
L4-5 9 mm
L5-S1 9 mm
Visualised SI joints are grossly normal.
Can you please help me out in telling the exact back problem I have and what could be the resolution and how it can be cured.Thanks in advance

Hi!
I have examined your query carefully, and studied the reports that you have sent us. Thanks for the details that you have provided in the query, they have been helpful.
1. I would ask you whether you have taken any medications for this problem, and whether you are taking any pain killers. Kindly mention, if you do have any chronic illnesses that require any medicines.
2. The MRI reports just indicate a straightening of the lumbar curvature. Note that this is a radiological artefact. From L5-S1 Junction onwards, the spine is normally curved, and abnormality that causes problems occur in curvatures such as lordosis, kyphosis and kypho-scoliosis. It is obvious, from the examination of your reports that indeed, there is no such issue.
3. Now, the commonest cause of the sort of problem that you are describing, lies in abnormalities of the discs of the spine. In this case, I would rule out a prolapse. Also, when the MRI was taken, your doctor indeed probably ruled out any pathology of the intervertabral discs. Further still, the dimensions of the discs are normal. Thus, we do not have any issue here.
4. And, I thank you for the clue that you have given to your diagnosis and the solution of your problem. You mention being a software engineer. It is statistically significant (by 2 deviations) that people who work on computers for the duration that you do have an occupational orthopedic hazard of 1) Lumbago and 2) Carpal Tunnel syndrome and occassionally, 3) Opthalmological issues.
5. The best solution, which is practical, and cheap is to get an "Air-FLO" chair to sit on. This is becoming very popular nowadays with many people working long hours on computers. This is a special orthopedically designed chair, just like an orthopedic matress, that augments the lower back. It also has special ergonomic support for the elbows.
6. "Air-FLO" is just one of the companies, making these chairs and you can purchase one from the brand of your choice.
7. Most doctors would give you a muscle relaxant and an analgesic. However, since I need to know the medications you are taking, we cannot make opinions on which medicines to take.

Note that this is no something new. Microsoft, has designed a special keyboard for people who had orthopedic issues with carpal tunnel syndrome. Again people, who had to work on computers for extended duration of time.

1)Currently taking this medicine as per doctor advise - Etoricoxib & Thiocolchicoside.
2) Do not have chronic illnesse or any other problem. First time got the back pain. Has been very active.
3) I have already taken new office ergonomic office chair which provide full lumbar support 2 months back.
4) I have been doing core strengthening exercises from past 3 months regularly. 30 mins in morning and 30 mins in evening.
5) I have been following the schedule one pain started in office to stand every 1 hour and do stretching exercises.
6) I only have a back pain or I will call discomfort near L4-L5 area. There are no shifting pains or leg pains or nerve pains as such.
Questions
1) DO you see instances of narrowing of spinal canal ? Spinal Stenosis ? Based on diameters?
2) I am working on very reputed position, so my concern is regarding my career to work as I am feeling difficulty to sit. So need to get this better.
3) I had various blood tests. New reports attached.
4) I want to know the exact/specific problem rather then a generic back pain so that I am aware of and further course of action.
5) I am looking for very specialized second opinion. Would appreciate in getting it from healthcaremagic.
6) I am ready to follow any schedule / precautions / lifestyle to get this sorted out.

1) There is no spinal stenosis. If there were, and it existed in the areas that you suspect, then most probably, the pain would develop into a neurogenic claudication of the limbs, in addition, to the sort of problems, that you are facing already.
2) I share your concern. The inability to sit for an extended reasonable period is disruptive to any sort of work.
3) I have examined your blood report. The counts are inconclusive of pathology meaning, a causative factor for back pain.
4) The specific diagnosis is repetitive stress injury.
5) I will refer you to an orthopedic super specialist on healthcaremagic. His name is Dr. Aashish XXXXXXX MS. 7) You can rewrite your question, add some details, think it over and then ask these 2 super specialists. It is a good idea to get a 2nd and 3rd opinion.
8) I am highly surprised, that you have been given very mild pain killers and medication. I would comment strongly here, that instead of the 2 drugs you are taking, they should be replaced with...
a) 1 muscle relaxant drug
b) at least 2 Painkillers, 1 NSAID high dose, and APAP combination.
c) Topical aceclofenac or diclofenac gel, with menthol for counter irritation.
All of my patients have gotten better, even those patients with hairline fractures, (chalk injury) with this sort of palliation. This combination of agents never fails to palliate back pain. In my experience of 10 years at least.
