What Does This MRI Report Indicate?

Question: I was experiencing lower back pain for almost 6 to 8 months and was suggested to get an MRI scan done for LS Spine.

The reports says:
-ANNULAR DISC BULGE WITH POSTEROCENTERAL DISC EXTRUSION AND ANNULAR TEAR AT L5-S1 LEVEL INDENTING ON THECAL SAC WITH NO NEURAL COMPROMISE.

Please advise for further medication.

I am Dr.Ajay Panwar,a neurologist, here to answer your query.

Posterocentral disc herniation with annular tear at L5-S1 can be managed with medicines and spinal extension exercises,however in some cases surgery may be required.

Please tell me some extra details-
1)Do you have any difficulty in passing urine and stool?
2)Do you have any weakness in legs, difficulty in walking?

Further, I shall be able to offer a better opinion if you can please upload your MRI image(focussing on L5-S1) or upload MRI CD on google drive in .rar archived format.
Medicines which are primarily used in such cases are-

Antineuralgics -Pregabalin,Duloxetine
Methylcobalamin
Calcium and vitamin D supplementation
A short course of NSAIDS-like etoricoxib, naproxen

I shall be glad to have you in follow-up with further details/MRI image.

Thank you Dr. XXXXXXX for a quick response. I am here with attaching the copy of MR.

Also, I am not facing any difficulty in passing urine or Stool.
No weakness in legs and while walking.

At time the pain arises from lower back (on one side only, left or right) and lowers to the foot. later after few hours it subsides automatically.

Thanks for being in follow-up.

I have gone through the MRI image which you have attached and can see the obvious disc extrusion.However, it would have been better, if you could have attached axial image at L5-S1.

Meanwhile, you can start on medicines as mentioned above and do regular spinal extension exercises.You should learn exercises from a physiotherapist with proper technique.

If your symptoms don't improve with medicines and spinal exercises, then neurosurgical opinion can be taken.
