I have gone through your question and reports and understand your concerns.
The MRI is showing small size ischemic changes which can be found with increasing age, diabetes, hypertension and other chronic illness .
The Cerebellar atrophy on MRI is to be read in light of present clinical symptoms .
If cerebellar features on examination is present then it has to be investigated.
The first approach will be to test vitamin B12, thyroid profile, drugs , toxin.
Degenerative diseases like spinocerebellar ataxia has to be considered if symptoms present.
Another possibility may be parkinsonism (multiple system atrophy) if symptoms present.
It will be easier to advice if you can provide some clinical and investigation details previously done.
