What Does This MRI Report Of Brain Indicate?

Question: Hi I wanted to send you this report on my mri. My doctor put down recurrent headache but I don't know why he did that because I have not had a migraine headache in at least 7 to 10 years. I use to get aura type headaches on occasion but not on a regular basis.
Findings on the report

There is no evidence of an intracranial hemorrhage , mass lesion, or acute infection.
There are tiny foci of high signal intensity on T2 weighted images in the deep white matter of the left parietal lobe as well as in the peri ventricular white matter of the right frontal lobe.
The ventricles and sulci are normal in caliber
The vessels of the anterior and posterior circulation demonstrate normal flow voids on T2 weighted images..
The parasail sinuses and mastoid air cells are clear.
Impressions - mild nonspecific white matter changes most in keeping with mild microangiopathic changes. A myelination process would be less likely but cannot be excluded. Similar white matter changes can be seen in patients with chronic migraines.

Having said all that from the report do you and I have not had a headache in years could this be put down to ms. My doctor is sending me to a neurologist soon I hope because getting to see a neurologist up in Canada takes soo much time and I am really worried.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your detailed MRI report.The white matter small vessel changes in MRI at your age may have the following common reasons-
1)Age related white matter changes(ARWMA)-These are usually normal and patients do not have much symptoms.
2)High blood pressure-Are you a hypertensive?
3)Diabetes and chronic smoking

Apart from these common causes,other pathologies can be-
Vasculitis
Chronic migraine

At this point,it is important to know about your symptoms.

If headache is the only symptom-then of course!Chronic migraine may have contributed to these changes and don't panic about them.
If you have a history of high blood pressure-it may be a definite cause and needs good blood pressure control.

Bottom line is-These MRI changes have to be viewed in light of neurological symptoms.If no neurological symptoms are there,these have a limited significance.

If you have some further questions,I shall be glad to have you in follow-up.

I have not had a headache in a long long time I have hypertension but it goes up and down sometime 115/75 sometime 135/95 but I do take blood pressure meds.
triglycerides are a little high and also vb12 and uric acid levels.
I do not smoke and no diabetes.
I was a heavy drinker 1.5 liters a day for a few years of white wine but have stopped that for the last 3 months now and will never have another drink again.
I just turned 59
I have a few pictures of my scans if that helps I can try sending, but do you think this has anything to do with ms

Sorry to keep bugging you but I am scared so I went to the hospital today where they did the mri and got the scan and report.

MS is not at all,in the scene.History of headache does not make MS,the diagnosis.Every white matter change does not mean MS.

I have gone through the MRI images you have uploaded but I am not able to make any inference just on basis of these images because you have uploaded either the basal cuts or superficial ones.Hardly,one or two pictures show a definite cut to look something on,but those cuts do not show any white matter changes.

It will be better if you can upload the MRI CD as a rar archived file on Google drive and paste the google drive link here in the follow-up question.

I shall have a look at your MRI CD,which will give an exact picture.

No luck this time too..Still the same problem over here.

Fine you can do one thing.Take good quality pictures of cerebral sections like the one (MRI-5.jpg) you have uploaded.Try to take pictures from all MRI films you have got.Other images you have uploaded (MRI-1.jpg,MRI-3.jpg,MRI-2.jpg,MRI-4.jpg)are either too superficial or basal cuts.

I hope you can do it easily.Still any confusion,please don't hesitate to ask.

I got well your files and had a close look at them.
Indeed,we have small ischemic white matter changes,which are predominantly periventricular.Some of these changes in left parietal deep white matter also.

These are very common in Hypertensive patients.Chronic migraine can also result in such changes.Keeping your clinical scenario,MS is not in question,beyond doubt.So,don't get so worried.

Another finding is - brain atrophy which is diffuse but more dominant posteriorly and in cerebellum.As you have corresponding clinical features,I presume it to be 'age related brain atrophy' which is physiological and normal.

Thanks so much for all you time a effort in looking at these problems I wish I could meet you sometime and thank you for looking into this.
One final question could these ischemic changes put me in greater risk for stroke or heart attack and dementia. I have been feeling quite depressed lately could this be attributed atrophy or concussions and or heavy drinking of white wine in the past.

Things are to be seen in clinical sense.These white matter changes don't appear to put you in a dangerous situation(like stroke or heart related problems).Nor, they appear to be a major risk for dementia.

Stop being depressed.You have got enough reasons for them(Hypertension and chronic migraine).So,try to control these factors and stop finding new reasons like concussions or depression.White wine does not have much of correlation with these changes.
