Few microvascular ischemic changes are common in your age group. Good control of blood pressure and cholesterol will lower the chances of more ischemic changes. Discuss about starting antiplatelet drugs with your doctors (I have seen few neurologist use them in patients with similar MRI changes).
In a nutshell, I don't see any significant MRI changes on your report. These changes definitely will not lead to dementia in the near future. You might need to consult an ENT surgeon to understand about the hearing loss.
