What Does This Abdominal Ultrasound Report Suggest?
Question: Dear Sir,
One of my relative (Male aged 72 years) has undergone the Abdominal Ultrasound Report and observations as below
1) Kidneys: Both the Kidney are normal in size and position
2) Right Kidney Measures 10.7*4.8 cms shows two exophytic cortical cyst. The cyst in lower pole measures 2.2*2.5 cms. Another cyst measures 2.4*2.5 cms.
3) Left Kidney measures 11.9*5.3cms shows complete loss of corticomedullary differentiation cortex replaced by multiple cyst of varying size. Largest of Cortical cyst measures 3.6*3.15 cms
4) Both the kidney show raised cortical echogencity
5) No evidence of hydroneophrosis/calculi
6) A defect of 2.3cns noted in anterior abdominal wall proximal to umbilicus with omentum hernitating through it. Cough impulse present. No evidence of obstruction at present
7) Liver - Normal in Size and shows normal echo texture. No evidence of focal lesion. No evidence of IHBR dilatation. Portal vein appears normal
8) Gall Bladder - Well distended wall appear normal. No evidence of calculi/sludge. No evidence of pericholic collection. CBD appears normal
9) Pancreas - Shows normal parenchyma. No evidence of ductal dilatation/calcification. No evidence of peripancreatic collection.
10) Spleen - Normal in size and shows normal parenchyma
11) Bladder - Well distended with 324cc of urine, post void urine residue 30 ml (not significant). No calculi. Bladder mucosa appears smooth.
12) Prostate - Mildly enlarged measures 2.8*3.7*4.1 cms volume 22.4 cc and shows normal parenchyma. No evidence of median lobe bulging into bladder.
13) No evidence of inflamed appendix
14) No evidence of enlarged lymph nodes
15) No evidence of free fluid
Impression:
1) Exophytic cortical cysts right kidney
2) Multiple cortical cysts left kidney with loss of corticomedullary differentiation
3) Bilateral grade 1 renal parenchymal changes
4) Grade 1 Prostatomegaly
5) A defect of 2.3 cms in anterior abdominal wall with omentum herniating through it. No evidence of obstruction at present
6) In view of multiple cortical cyst in both kidneys please rule out a poly cystic kidney disease. Regular follow is adviced
We request you to please advice the course of action we should take. We are worried about this.

We have undertaken C.T. KUB (Plain) Test and Report is appended in the mail.
Axial Sections of Abdomen Reveal -
Simple Renal Cyst - Bilaterally
28mm lower pole right kidney
Multiple cysts left side dominant one 32 mm, one each in upper and midpole left kidney.
Both Kidneys, size, position contour normal. Renal Parenchymal volume maintained. No focal lesion/mass. Cotrical thickness and cotrico-medullary differentiation maintained. Central renal sinus appear normal. Aorta and do not reveal any focal stenosis. Renal hila normal. No obvious mass/enlarged nodes
Right Kidney measures: 90 x 43mm
Left Kidney measures: 130 x 57mm
Urinary bladder normal. No obvious wall/intraluminal pathology. No vesical calculus. No obvious calculus/dilatation noted at lower end/vesico-ureteric region.
Liver, Gall bladder, Spleen and Pancreas are normal.
Impression:
C.T Scan reveal -
Simple Renal Cysts - Either Side
No other focal lesion/mass
No Obvious calculus seen in pelvicalyceal system/ Entire course of ureter
No Hydronephrosis/Hydroureter
No Obvious Vesical calculus nor any wall/intraluminal pathology.
Reports were showed to Doctor (Urologist and Nephrologist) and it was informed that due to regular pain killer tablets, the cysts were increased and creatinine levels were on a higher side (2.09) (since the relative is suffering from filariasis.). It was also adviced to completely stop pain killer tablets.
Your opinion and advice is very much anticipated.

1. I don't see any new findings on the CT scan report other than in the Ultrasound scan.
2. I agree with your doctor that regular pain killers affected the kidney function tests(Creatinine - 2.09). Do not take any more pain killers as instructed by doctors.
3. The Cysts and the kidney functions should be regularly monitored by a Nephrologist.
