default
Posted on Mon, 19 Jan 2015
Question: Thanks for the wishes! Yes, Biopsy has already been done about two months back (in Oct.) in my hometown (Hisar/Harayana) during our stay there on Diwali vacations.
Since Biopsy report was inconclusive, Biopsy Surgeon referred for Ultrasound guided FNAC for further diagnosis and concluded TB.
Please find the Biopsy report enclosed.
We are staying in XXXXXXX
Please advise us the best possible direction as we are extremely worried.

Brief Answer:
Continue AKT4 and consult XXXXXXX Asia hospital.

Detailed Answer:
Hello dear,

Thanks for your follow up question.

No need to worry much as lymphnode tuberculosis is not much of a severe disease.
So start AKT4 as advised by me.
And in XXXXXXX you can consult XXXXXXX Asia hospital. It has a good tuberculosis unit.
Or you can consult other tuberculosis specialist in XXXXXXX
And no need to worry much. She will be fine.
Continue AKT4 and benadone tablet.

Thanks for the answer.

Pls Pls Pls.... Need your opinion on the basis of info shared so far. Let me sum them up here in chronological order as they transpired:
//Early Sept : Wife discovered small lump under right neck.
//Oct Mid : Lump got swelled quickly and caught our attention.
//Oct last : As we were in our hometown (haryana), we visited local Surgeon and asked to undergo Blood, Urine, ESR, Regular needle FNAC Test. Except High ESR, rest of the results were negative.
//Oct last : Visited another renowned TB specialist and undergone, Chest Xray, TB skin test, again needle FNAC. Except Skin test which was partial positive, everything else was negative. Referred for Biopsy.
//Oct last: We reached swiftly, and through a known surgeon (cousin brother), Biopsy got done. Waited for a week for the results. Outcome was inconclusive/Negative with no malignancy found. Surgeon/cousin bro. further referred for Ultrasound guided FNAC and results were positive, so concluded TB.
Apart from that, stomach ultrasound, USG whole abdomen, LDH was also carried out and result was negative.
// Early Nov: We reached back to our work place XXXXXXX and thought of taking opinion here as its a long treatment and need continuous doc involvement. Here underwent Chest and neck CT scan and outcome was negative. Based on the previous Tests, we were advised to start TB treatment and now its been two months.

After going through all these tests, and continuing medicine for two months, their is no improvement. I am looking forward for your honest opinion on the basis of above stated facts. Please show us the correct path towards recovery or further diagnosis if needed.

Best Regards

Brief Answer:
no need to worry much.

Detailed Answer:
Hello dear, thanks for your follow up question.
Now things are clear.
I advice to give her AKT4 simultaneously. Also give benadone tablet.
Give these for 1 month.
The swelling will take time to decrease.
And since 2 months, she was not on standard treatment. So response of treatment is not seen due to inappropriate regimen.
So take whole AKT4 pack togather for 1 month.
And update me her progress after one month.
Hope I have solved your query.
