Question: Total chloresterol is 235 ...ldl 168.. hdl 39 ...triglycerides 140 and hemoglobin 5.7% what do these really mean

Greetings and welcome to HCM. I understand your concern.

Basically, this means that you have high blood concentrations of cholesterol and LDL-cholesterol. The other data are within normal ranges.

Although I need only one clarification: the parameter "Hemoglobin" you mentioned, is it the Hemoglobin level or the Hemoglobin A1C level? It is normal if it is the latter. Since high cholesterol and high LDL fraction are independent risk factors for developing coronary artery disease, there is a goal that measures should be undertaken to lower these figures. I can see you tried diet and exercise, which are very effective measures to achieve such a goal. Anyway, I would once more provide my opinion:

- 60 minutes a day of walking, jogging, biking etc.
- green leafy vegetables and fruit up to 300 g a day
- chicken or other poultry and fish, not red nor raw meat. The latter may be used maximum once in two weeks

- reduce alcohol intake to a maximum of 30 ethanol a day, i.e. a glass of wine
- the white of the egg, not the yellow fraction

Also, since these or other measures have been undertaken for three years and still the resulting cholesterol levels are higher, a Statin (hydroxy-methyl-glutaryl coenzyme A inhibitor) should be started on your therapy. Whichever from this class of drugs is effective and safe to use with the aim to lower cholesterol levels.

The only matter is that these drugs may elevate liver cell parameters (AST, ALT). Therefore, three months after you have been on the therapy with a statin, I would recommend you that, besides cholesterol, also these parameters should be measured.
