What Does This Blood Report Indicate?

Question: Hi ,

I have a family member with history of heart problem and High pressure during the day. Also even thought nutrition is good, cholestérol, b12 Level, d-dimer are very High. Although, diastolic seems to be the only thing going up. Pulse and systolic seems to be stable... Would there be an explanation to this phénoménon? See results attached. Thank you

I carefully passed through your question and reviewed your performed tests and would explain that D-dimmer is not a specific lab test, which can result altered in many disorders.

This test is usually used to detect for possible deep vein thrombosis, stroke or pulmonary embolism.

But, you should know that D-dimer levels may be elevated in many other circumstances like: in the setting of pregnancy, inflammation, malignancy, trauma, postsurgical treatment, liver disease (decreased clearance), and heart disease. It is also frequently high in hospitalized patients.

From the other side, high triglyceride level, an elevated bilirubin level, an elevated serum rheumatoid factor level, or hemolysis may falsely increase the D-dimer level.

So, coming to this point, I would recommend making further tests to investigate for the possible cause of D-dimmer increased levels:

- a chest X ray study and respiratory function tests to exclude possible lung disorders
- a cardiac ultrasound to examine her cardiac function and structure
- a Doppler ultrasound of the leg vessels
- an abdominal ultrasound
- some other blood lab tests (complete blood count, PCR, sedimentation rate, kidney and liver function tests, blood electrolytes, Rheumatoid factor levels, fasting glucose).

Regarding his blood pressure, besides performing the above tests (which also help to exclude other possible secondary causes of high blood pressure), some tips would be helpful:

- reduce salt and caffeine intake
- perform a lot of physical activity
- reduce weight if she is overweight
- avoid smoking

You should discuss with her doctor on the above issues.

What about high b12 and high ferritin level ? Is this an important element ?

Yes, high vit b12 and ferritin levels could also be a sign of the metabolic disorder.

An increased vitamin B12 level is uncommon. Usually, excess vitamin B12 is removed in the urine.

Conditions that can increase B12 level include liver disease (such as cirrhosis or hepatitis) and myeloproliferative disorders (for example, polycythemia vera and chronic myelocytic leukemia)

Elevated ferritin levels are usually due to causes such as acute or chronic inflammation, chronic alcohol consumption, liver disease, renal failure, metabolic syndrome, or malignancy rather than iron overload.

So, I recommend discussing with your doctor on the above recommended tests, in order to find the possible cause of these metabolic changes.
