What Does This Bone Marrow Biopsy Report Indicate?

default

Question: I already asked one doctor his opinion, so this is a second opinion and a few more questions. He was really great, so this is no reflection on him. (If this is you again Dr. Sundriyal I hope you don't mind if I get a second read on my situation)

I had low wbc on 2 consecutive tests. Both had very low absolute neutrophil count. Additional testing has showed 4% myelocytes and 1% metamyelocytes as well as 3% atypical lymph, slight poikilocytosis and +1 polychromasia. Lymphocytes 65%. LDH 283. Sedrate 38, CRP 2.3

I had a bone marrow biopsy and have uploaded the file with the results. My most recent CBC (same day as bone marrow) showed a WBC at 5.1,

They have already ruled out B-12, Folic acid deficiencies, HIV, Lupus, Rh, hepatitis.

1) Why does the marrow report say abnormal? Is it basically fine?
2) I don't understand how I was not sick (no sore throat, stomach issues, etc) and had such abnormal blood results. I have many serious illnesses and viruses including emergency mastoiditis, Epstein Barr, CMV and never had any abnormal bloodwork except high WBC with infection. Is a recovered WBC and this bone marrow report basically saying I'm fine?

3) Is the M:E ratio normal?

4) Is the cellularity normal?

5) Is it no big deal that there are rare dysplastic megakaryocytes?

Thank you!!

Brief Answer:
Bone marrow is within normal limits

Detailed Answer:
Hi
Thanks for your query.

This bone marrow report does not raise significant concerns. Please find below my answers to your queries..

1. Probably says abnormal because of the variable cellularity and rare dysplastic megakaryocytes

2. Maybe it was a mild viral infection which suppressed the marrow transiently. Recovered blood WBC count and a near normal marrow suggests recovering marrow.

3. Yes, the ME ratio is normal

4. The cellularity overall is normal but it varies from low to normal, probably because of a regenerating marrow.

5. No, rare dysplastic megakaryocytes are not of concern.

Overall, things are on the right track.
Hope this helps.
Regards
