1. In most patients these tiny bright spots are non specific, meaning that their cause is not yet established as they do not follow any particular pattern and are not a serious concern.
2. It is difficult to point out any food, medication, toxic substances or environmental factors that might be responsible. It is likely that nothing of that sort has caused these findings. Therefore you do not require to refrain yourself from the small pleasures of life like eating and sporting.
3. One possibility is that these might be signs of slightly early changes. It will help to periodically get your blood pressure and general health evaluated so that any condition might be recognized early and treated. In short there are tiny areas in brain where the blood flow was at one time deficient and therefore negligible ischemia has occurred. There is no requirement to take any vasodilator if no condition is evident clinically. Putting hot compresses on the head is not a good idea. You might participate in mind relaxation techniques and this might increase the blood flow in the brain, especially around the possible deficient areas.

Answer... My mother died in my arms 3 month ago from a cerebral hemoragea....i think it is normal i need a little more info with the mri finding i received....i also do a lot of meditation but cant do sports anymore because of extreme stiff and

2. Your neck problems are due to the disc degenerative changes in the cervical spine at levels C 3 - 4 and C 4 - 5 levels and this might cause some symptoms. I have seen this in your earlier MRI scan pictures.
3. Since your mother had a cerebral hemorrhage, please get a clinical evaluation done by the cardiologist to know any risk of hypertension and heart disease. There are risk assessment methods and these will predict any risk and your doctor will guide you through regular tests. It is good to know that you do meditation and mind relaxation exercises. Please maintain your stress levels at lowest possible amounts.

Doctor here dont think anything relevant is going on with my neck degenerescence..... I would put some gel voltaren on the neck but there is a increased risk of stroke while using diclofenac... What would you recommend appart from relaxation and physiotherapy ? Is there ways to stop this and be back at sports someday or i am already doomed with the neck... ? Could that degenerescence in the neck be caused by an organism ? Like a small osteomyelite due to fungus or something else ?

1. I too find it strange that all of a sudden a gentleman aged 32 years is having uncontrollable shaky hand with sudden lost of coordination and memory as you have mentioned in your earlier query.
2. It will help to know if you had any nasty fall or lifted heavy weights during sports. Having early degenerative changes are seen in few individuals and I had a doctor friend of mine whose X ray at 40 years age appeared to have developed degenerative changes expected in a 60 year ole man.
3. For your condition, you can continue to use diclofenac gel. The stroke risk is not a concern when using the gel. You might also consider using over the counter spray and ointment like Salonpas which contains Menthol and Methyl Salicylate which might provide you relief.
3. Using hot and cold packs on the neck and lower back might also provide comfort. Muscle spasm stays for about 3 to 5 days. Relaxation and physiotherapy are the better options should you not get relief after using gel.
4. Degeneration cannot be stopped but it can be slowed down by using precautions. Once you are symptoms free after doing a rehabilitation then you might consider participating in sports which will not cause any pressure on the neck.
5. Your symptoms are unlikely to be due to any organism, osteomyelitis or fungus and any thing else. Your MRI scan shows mild degenerative disc changes which might be causing pinching of nerves.

You are a very skilled and reassurant doctor Mr XXXXXXX I appreciate your help very much. Since i had a VZV / EBV reactivation not long before all these symptoms started could it be related to a viral infection as in a kind of post herpetic syndrome ? Maybe the nerve pain / heart palpitations / lymph node / and all symptoms could be somewhat triggered by a viral infection as it was never treated arv's.....

3. Varicella zoster reactivation is a strange condition and the rashes disappear in a week but the pain might persist for a few months. There is a strong possibility that your discomfort might be the result of herpes reactivation in recent past.

1. One should start acyclovir or valacyclovir within 3 days after the rashes start. Delaying valacyclovir after that will have no effect on the disease progress and duration of rashes.
2. The only way to treat the symptoms persisting after varicella zoster is by providing adequate pain relief.
3. Antibodies are present in every person for past infection and once the rashes have appeared then you are sure that it was varicella zoster

1. The varicella zoster virus might affect brain tissues but your brain scan does not show any changes due to possible varicella zoster infection.
2. Please write back with the areas in which you had rashes. The nerves in that area is most likely to be involved causing symptoms.
3. The pain is a benign but disturbing symptom. It does not cause any serious complication and will not affect any system permanently. However it causes discomfort which might require strong pain relief medicne. You can ask any question that you have and I will try and answer it.
