What Does This Following Blood Report Indicate?

Question: Hi Doctor
I am 34 Year old man working in IT field.
Recently I met a doctor for fever related checking. After that continued my health checkup and found the issues in Vitamin D deficiency and difference in SGPT and SGOT values. So doctor suggested to have udiliv-300 and essentiale-L 1 and 2 respectively on daily. Now above values are coming to normal and now Vitamin D - 54mg and SGPT - 118 and SGOT - 75.
Also I have another problem of heart burn. So doctor suggested me to have rabilife tablets(1 tablet/day)
So kindly let me know how long i have to continue these medicines

I'm Dr. Shuba Hariprasad and will be answering your query today.

Would you clarify a few things for me please?
*how much were your sgot, sgpt, TSH values earlier?
*what was your diagnosis when you had the fever?
*do you smoke or consume alcohol?
*what is your height and weight?
*do you eat on time?
*how much water do you drink a day?
*do you have any other medical issues or take any other medications?
*how much of thyronorm were you on before taking the latest test (5.25)? And for how long?
*n what other tests or procedures have you had?

I recommend :
-stop vitamin D supplements.
# get about 30 minutes of sunlight a day. (before 10am or after 3pm).expose arms, neck and face. Don't use sunscreen at this time.
# have eggs, milk for natural sources of vitamin D
#recheck levels after 6 months.

- stop alcohol and smoking

- continue other supplements for 2 more months. Then stop.
#recheck sgpt and sgot after a month of stopping meds

- regarding thyronorm, I will advise after hearing back from you.

-rabilife-take for three days and stop.
#prolonged intake of Proton pump inhibitors (rabilife) will cause more harm than good in the long run.
#eat smaller and frequent meals (every 3 hrs) and don't skip meals
#drink plenty of water (about 3 liters a day)
# avoid coffee. Have tea with meals and not on an empty stomach. Limit tea to 1 cup a day.
# have healthy snacks (sprouts, nuts and seeds) instead of fried items
#have homemade foods. While eating out, stick to steamed or boiled foods.

These measures should help with heart burn and also promote overall health.

Hello Sir,
Thank you for the follow up and clarifications.

1)I want to confirm the latest TSH report.
In the reports that you have attached with this question, your TSH is 0.2 (taken on 06/11/16).

You mentioned 5.25. When was this done?

If the above is the latest, then you need to reduce your thyronorm dose to 12.5mcg /day.

If 5.25 is the latest then you need to take 50mcg every day.
These doses are based on the doses you mentioned (25 and 50mcg alternative days).

2) what did you do differently after 16/10/16? With the same medications, your sgpt and sgot have gone up?
Any alcohol intake or illness?
Have you done an ultrasound of the abdomen?

3)was the H. Pylori test done at a lab or in the doctor 's office?
Are your symptoms better after taking rabilife?
Have you had any antibiotics to treat the H. Pylori infection?
Have you had an gastric endoscopy? (if yes, what were the results?)

Rabilife should not be taken indefinitely. (will affect kidneys). The cause of your dyspeptic symptoms needs to be evaluated. If your symptoms are not better after 4 weeks of rabilife an endoscopy( to check for ulcers and gastroesophageal reflux disease - GERD) needs to be done.

Please consider my previous recommendations regarding your diet to minimize symptoms of dyspepsia (heart burn, stomach pain).


Continue your other supplements and liver protective medications for another 2 months and check again. Thyronorm as advised above.
Check TSH every 6 weeks
Other tests are normal.

Thanks for your suggestions. Please find the following reply

1)I want to confirm the latest TSH report.
5.25 is correct value and report on 12-11-2016.
0.25 is wrong report on 06-11-2016 mistakenly taken after having food.



2) what did you do differently after 16/10/16? With the same medications, your sgpt and sgot have gone up?

after completed the first section of medicine (3 weeks) report was good.
but I missed two days tablets before starting second session.
I just had wine two time in between second session.

Have you done an ultrasound of the abdomen?
Yes, ultrasound report is normal and no issues found.




3)was the H. Pylori test done at a lab or in the doctor 's office?
Yes, from hospital

Are your symptoms better after taking rabilife?
Yes, but if I am stopping then i feel symptoms increased.
Example: before medication i didn't have too much problem with foods. after alcohol consumption(more quantity) only i feel such issues. And this will continue two three days.
Also had issues with some foods like eating banana or more halwa etc.

But now If I am stopping rabilife I feel more issues. But if taking rabilife the no more issues found.

Have you had any antibiotics to treat the H. Pylori infection?
No

Hello Sir,
Thank you for your clarifications.

I suggest ;

1)Please take 50 mcg of thyronorm a day. Repeat TSH after 6 weeks on an empty stomach.

2)please do an endoscopy to check for ulcers. Please stop alcohol. Rabilife is not a cure and only treats the symptoms.
If any ulcers are found, then you will need to take a course of antibiotics to treat it. Rabilife is also prescribed along with this.
After treatment, a repeat test of h. Pylori IgG is required. If still positive after a year of initial IgG test, then repeat treatment course with different antibiotics is required.

Please try my recommendations regarding diet modifications and water intake. It will help.
