Brief Answer:
Hepatic and urinary copper study is further needed

Detailed Answer:
Hi,

I have carefully read your query and understand your concern.

You seem to have low levels of serum ceruloplasmin leves ie 18.9 mg% .

The normal reference values are 20-40mg%.This should raise strong suspicion of Wilson's disease in a young patient below 40 yrs of age who present with unexplained chronic liver disease.

The stage 2 fibrosis cannot revert back to stage 1 f as the process is progressive and irreversible.

At this moment I can all say that it is a non curable stage of the disease irrespective of the diagnosis.

I can comment further if an accurate confirmatory diagnosis is made based on two more further tests of Hepatic copper concentration obtained through biopsy and urinary copper excretion.

I strongly recommend you to get these tests at the earliest.

Elevated SGPT and SGOT with gall stones can be a part of the pathological process of liver fibrosis/cirrhosis-f2/3 stage.

-ve ANA can rule out autoimmune liver disorder quite comfortably.

So I would like you have those above mentioned 2 tests and have final discussion on further treatment protocols and disease course, once they are obtained and posted on your follow up query.

Thank you

Brief Answer:
An averge of 2 years

Detailed Answer:
Hi Sir,

Progression to f4 depends on various factors.
Any metabolic or new insult to liver can hasten the process if not timely treated.

On an average,it takes around 2 years to progress to f4 in patients with mild to moderate symptoms.

Thank you

Brief Answer:
vital nutrient rich diet is preferred

Detailed Answer:
Hi Sir,
Delaying the progression can be a big challenge.

Avoid infections and polluted environment as far as possible.

Treat any infections at the earliest.

Completely avoid alcohol and smoking.

Good diet is the corner stone in the management of liver fibrosis/cirrhosis.

Any such intervention can prolong the life by 5 years.

Few life extension suggestions based on nutrient supplementation.
1.Branched chain amino acids 2400 mg/d
2.milk thistle 750 mg/d
3.L glutathione -500 mg/d
4.N acetyl cysteine -1200 mg/d
5.vit c -600 mg/day
6.vit e -300 mg/d
7.selenium -200 mg/d
8.vit b12-1000mcg/d
9.pantothenic acid-1000mg/d
10.choline -45 mg/day.

Thank you
