Regarding your concern, I would explain that a positive tilt table test would indicate the presence of orthostatic intolerance.
Usually in most of the cases no underlying cause is found. Nevertheless, further tests to investigate for the underlying possible cause may be needed:
- cortisol plasma levels for possible adrenal gland dysfunction
- anti ganglionic antibodies for possible autoimmune disorder
- Dopamine beta hydroxylase plasma levels for possible enzymatic deficiency
- complete blood count for anemia
Regarding the treatment, I would recommend:
a) non pharmacological treatments
1- diet modifications (increase salt and caffeine intake, take plenty of fluids, avoid large meals and try to eat more frequently in small meals)
2- Equipment aids may be helpful. These include tight support stockings or antigravity suits.
3-Perform positional changes, such as standing up, slowly and gradually.
4-Elevate the head of the bed and avoid prolonged recumbency.
b) pharmacological treatments to be considered:
1- Fludrocortisone
2- Midodrine
3- Droxidopa
4- Desmopressin
If any underlying cause is revealed, treatment of the underlying cause is also necessary.
