Her urine report is not at all suggestive of UTI. But her Ultrasound report shows cystitis that is supportive of UTI.
Now here, what is required is clinical co relation.
Also she is a diabetic so chances of UTI are more in them.

2. If fever is there then tell me since when ? Associated with chills and rigors? Any burning micturation?
3. Her blood sugar levels?
4. What treatment she is on?

-thyroiditis has almost been confirmed after fnac and thyroid scan
-fever with chills.fever comes at around 6:30 to 7:30 pm
-last hba1c 25 febuary=6.2%,
-today pp blood sugar=206 mg /dl

In this case, i must say that fever can be due to Thyroiditis.
In such situations i prefer doing the following:
1. Send Urine for Culture Sensitivity.
2. Start patient on oral antibiotic like Levofloxacin, Nitrofurantoin to cover urine infection part, after sending culture sample.
Your report would be available in 48 hours, so if there is a growth in culture then antibiotics should be continued according to the sensitivity, else can be stopped.
Get her a good treatment for thyroiditis and also she requires a better control of her diabetes.

-does an enlarged liver suggests any negative thing because for past 2 years all ultrasound report has enlarged liver.
-doc also told to get x ray pa view what does this has to do with the current medical medical problem.

Enlarged liver warrants further investigations to see for Viral Hepatitis or Non Alcoholic Fatty Liver disease.
You should simply get her following investigations:
1. HbsAg, Anti HCV Antibody
2. Liver Function Test.
X-ray Chest he must have asked to rule out any evidence of Pneumonia that might be causing Fever, as in diabetics any infection can occur most common being UTI and Pneumonia.
In fact simply, get her all routine tests once, so that if any problem is there that can be identified and treated.

Urine Culture Report is negative, means there is no growth.
I don't feel that fever is because of UTI. Moreover, if it is, it must have responded to antibiotics.
Urine culture can be negative even after UTI, if antibiotics have been started before giving the urine sample.
I still feel that fever is because of thyroiditis.
I would like to know:
1. Current status , including fever episodes, how high does the fever rise?
2. What treatment she is on?
3. Any other symptom then fever like cough, expectoration?
4. Also attach her thyroid profile and FNAC Report.
Reply with these so that i can give you a better opinion.

1. no fever as she takes dolo 650 mg tablet 3 times a day.the fever has now stopped coming but as far as the pattern is concerned it used to come between 6:30-7:30 pm on daily basis and then the fever goes of the next morning.fever was a bit on higher side.
2.Amaryl 1 mg,volix m,shelcal m,matilda forte,dolo 650 mg,ciplar 10mg,
3.no other symptom but yes she got fever with chills.

If she still has fever, specially evening rise, get her Chest Xray and Sputum for AFB to rule out Pulmonary Tuberculosis.
You have not mentioned if she has been given any antibiotics.
Also, i feel that she has a fever of long duration now. So, get her investigated again and properly.
Her blood sugars are not under control. Discuss with your doctor about adding Metformin to her treatment for a better control of blood sugars.
Also get her thyroid profile again.
